三种血液病患者的红细胞可变形性

摘要 作者应用激光衍射法测定缺铁性贫血(IDA)8例、再生障碍性贫血(AA)8例及骨髓增生异常综合征(MDS)13例共29例血液病患者的红细胞可变形性变化.血样采自肘静脉,肝素抗凝.将衍射图拍照,测量图形的长轴和短轴,计算出在3种相应切应力(54dyn/cm^2,166 dyn/cm^2和309 dyn/cm^2)作用下的红细胞长轴,长轴被拉伸得愈长说明变形性愈大.结果表明,正常成人组在上述3种切应力作用下的红细胞长轴分别为13.267±0.057μm,15.238±0.585μm和17.163±0.883μm.在3种切应力作用下,3种疾病的红细胞变形性都降低:与正常组相比,IDA组均为P<0.01;AA组及MDS组者在低切应力时P<0.01,其余P<0.05.3种疾病红细胞变形性降低的顺序为IDA>M0S>AA,且在低切应力时降低最明显. The erythrocyte deformability of 29 cases in iron-dificiency anemia (IDA), asplastic anemia (AA) and myelodysplastic syndrome (MDS) was studied by ektacytometry, a laser diffraction method. The blood samples from albow veins were heparinized. Three fluid stresses acting on red blood cells (RBCs) were 54 dyn/cm2, 166 dyn/cm2 and 309 dyn/cm2 . The changes in laser diffraction patterns from circularity to ellipticity were photoed, and the long and short axises of RBCs under the effect of the corresponding shear stress was calculated. The longer the axis of RBCs, the greater the deformability. The results showed that the calculated long axises of RBCs were 13.267±0.057μm, 15.283±0.585μm and 17.163±0.882 μm in healthy adults. Compared with the healthy adults, there was a reduction of erythrocyte deformability in the three hematological disorders: P<0.01 under each of the three stresses in IDA group, P<0.01 under lower shear stress and P<0. under the other two stresses in AA group and MDS group. Therefore it is concluded that the degree of the reduction is in the following sequence: IDA>MDS>AA, and the reduction of deformability is the most obvious under the lower shear stress.
