摘要
采用临床和动物实验模型,观察肢体不同缺血、不同再灌流时间下,患肢股静脉回流血中肌酸磷酸激酶(CPK)的动态变化,并辅以肌组织病理检查。实验结果表明:(1)缺血和再灌流损伤与缺血时间明显相关,缺血时间越长,其损伤越重。(2)再灌流期的病理改变明显重于缺血期,主要以肌组织充血、水肿和大量炎性细胞浸润为其特点。以再灌流后16~32h最明显。(3)肢体缺血1~30h,其缺血损伤不显著,但再灌流后24~72h出现明显的再灌流损伤。因此,应引起临床的高度重视。
The changes of creatine phosphokinase activity in the different periods of ischemia and reperfusion of extremities were observed in clinical cases and laboratory models.In addition,the pathological changes of the skeletal muscles were also studied.The findings were as follows:(1)The severity of ischemic and reperfusion lesions was dependent on the ischemic duration,extent and severity.The longer the ischemic duration,the larger the ischemic lesions;(2)The pathological changes were more severe in the reperfusion period than in the ischemic period and they were characterized by congestion,edema,swelling and infiltration of large amounts of inflammatory cells in the skeletal muscles.These manifestation were most marked from the 16th to 32nd hour after reperfusiont(3)When the ischemic duration was less than one to one and half hours,the resultant lesion of little significance.But the lesions were quite severe when the ischemic duration was 24 to 72 hous,and the lesions in the repufusion period were also severe,which needs specical attention from the surgeon.
