摘要
目的分析扩张型心肌病(dilated cardiomyopathy,DCM)的临床文献,探讨其中医证候演变及分布规律。方法通过检索中文文献检索平台数据库收录的、有关DCM的中医学文献资料,提取其中临床症状体征、中医辨证分型及病程等信息并进行相关统计分析。结果共筛选并纳入文献271篇,涉及病例11577例。发现DCM相关证素12个,以气虚、血瘀为主;病位以心、肾为主。提取DCM临床证候70个,以气虚血瘀证、气阴两虚证、心气阳虚证、心肾阳虚证、血瘀水停证最常见。结论DCM病性虚实夹杂,病位在心,涉及五脏,与肾关系最为密切;其中医证候多见虚实夹杂证,以气虚血瘀证、气阴两虚证、血瘀水停证、阳虚血瘀证、气虚痰湿证、阳虚水泛证为主。
Objective To analyze the clinical literature on dilated cardiomyopathy(DCM)and explore the evolution and distribution regularity of its traditional Chinese medicine(TCM)syndromes.Methods By retrieving the TCM literature on DCM included in the da⁃tabases of Chinese literature retrieval platforms,Information such as clinical symptoms and signs,TCM syndrome differentiation and typing,and disease course was extracted,and relevant statistical analyses were conducted.Results A total of 271 articles were screened and included,involving 11,577 cases.Twelve syndrome elements related to DCM were identified,among which qi deficiency and blood stasis were predominant,and the main disease locations were the heart and kidney.Seventy clinical syndromes of DCM were extracted,with qi deficiency and blood stasis syndrome,qi-yin deficiency syndrome,heart qi-yang deficiency syndrome,heart-kidney yang deficiency syndrome and blood stasis with fluid retention syndrome being the most common ones.Conclusion The nature of DCM is a mixture of deficiency and excess.Its disease location is in the heart,involving the five zang-organs,and it has the closest re⁃lationship with the kidney.Its TCM syndromes are mostly mixed deficiency-excess patterns,mainly including qi deficiency and blood stasis syndrome,qi-yin deficiency syndrome,blood stasis with fluid retention syndrome,yang deficiency and blood stasis syndrome,qi deficiency with phlegm-dampness syndrome and yang deficiency with fluid overflow syndrome.
