摘要
介绍邓铁涛教授以调脾护心为法治疗扩张型心肌病心力衰竭的经验。本病病位虽在心,但与五脏相关,尤重于心脾;本质为本虚标实,心气血阴阳亏虚于内,致使痰瘀水停之标遏阻心阳,故急性期常按阴阳分治,在温阳或者滋阴的同时均强调温补脾胃阳气;缓解期则需综合调理,注重以调理脾胃为先,才能有效防止复发,带病延年。在遣方用药上补虚泻实,灵活应用气血同调、攻补兼施、升降相随等原则。
This article is to introduce Professor DENG Tietao＇s experiences in treating dilated cardiomyopathy heart failure with regulating spleen and protecting heart method. The location of the disease is at heart, but it is relat- ed to five Zang-viscera, especially to heart and spleen. The nature is deficiency in origin and excess in superficiality. Internal deficiency of heart Qi, blood, Yin and Yang leads to phlegm, blood stasis and water retention in superficiali- ty, which inhibits heart-yang. Tiros in acute stage, the disease is treated based on Yin and Yang. At the same time of warming Yang or nourishing Yin, warmly invigorating spleen-stomach Yang-qi is emphasized. In remission stage, comprehensive regulation is needed. Prof, DENG emphasizes that regulating spleen-stomach first may prevent recur- rence effectively to prolong Survival. As for composing prescriptions and using herbs, Prof. DENG tonifies the defi- ciency, purges the excess and flexibly applies the principles of regulating both Qi and blood, reinforcement and elimi- nation in combination, as well as ascending with descending principles.
