摘要
目的探讨口服肠内营养制剂在高强度聚焦超声(HIFU)治疗子宫肌瘤术前肠道准备中的安全性及可行性。方法选取2023年1-12月在自贡市第四人民医院确诊子宫肌瘤并接受HIFU治疗的140例患者为研究对象,随机分为研究组(术前口服肠内营养制剂)和对照组(常规肠道准备),每组70例。比较2组HIFU治疗相关指标、生化指标,以及术前、术后(术中)疼痛评分、口渴评分、饥饿评分、并发症等。结果研究组的HIFU辐照时间、治疗强度及治疗剂量低于对照组,术后前清蛋白水平高于对照组,围术期输液量少于对照组,差异均有统计学意义(P<0.05)。2组建立超声通道所需时间、HIFU治疗时间、子宫肌瘤消融率、HIFU术后禁食时间、住院时间、住院费用及并发症比较,差异均无统计学意义(P>0.05)。研究组术后疼痛评分、术前及术后口渴评分、术前及术后饥饿评分均低于对照组,患者满意度评分高于对照组,差异均有统计学意义(P<0.05)。结论HIFU治疗子宫肌瘤术前口服适量的肠内营养制剂安全可行,可改善患者治疗体验。
Objective To evaluate the safety and feasibility of using an oral enteral nutrition formula for preoperative bowel preparation for high-intensity focused ultrasound(HIFU)ablation of uterine fibroids.Methods A total of 140 patients diagnosed with uterine fibroids who underwent HIFU ablation at Zigong Fourth People's Hospital from January 2023 to December 2023 were selected as the study subjects.They were randomly assigned to either the study group(receiving preoperative oral enteral nutrition formula)or the control group(receiving conventional bowel preparation),with 70 patients in each group.HIFU ablation-related parameters,biochemical indices,as well as preoperative and postoperative(intraoperative)pain scores,thirst scores,hunger scores and complications were compared between the two groups.Results HIFU sonication time,treatment intensity and treatment dose in the study group were significantly lower,while postoperative prealbumin level was significantly higher and perioperative fluid infusion volume was significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The time required for establishing the acoustic pathway,HIFU ablation time,ablation rate of uterine fibroids,postoperative fasting time,length of hospital stay,hospitalization costs and complications showed no significant differences between the two groups(P>0.05).Postoperative pain scores,pre-operative and postoperative thirst scores,and preoperative and postoperative hunger scores in the study group were significantly lower,whereas patient satisfaction scores were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Preoperative oral administration of enteral nutrition preparations is safe and feasible for patients with uterine fibroids undergoing HIFU treatment,and effectively improves the treatment experience.
