摘要
目的探讨中药熏洗联合高强度聚焦超声治疗湿热下注证外阴白色病变的临床效果。方法回顾性分析2021年12月至2022年12月收治的湿热下注证外阴白色病变患者的临床资料。按治疗方法分为超声治疗组97例和联合治疗组103例。超声治疗组应用高强度聚焦超声治疗,联合治疗组应用高强度聚焦超声联合中药治疗。比较两组临床效果、不良反应,比较治疗前和治疗1个月后皮肤症状改善情况(外阴瘙痒、外阴色素脱失、阴痛)炎性因子水平[超敏C反应蛋白、白细胞介素-6、降钙素原]和生活质量[皮肤病生活质量指数(DLQI)],统计两组治疗后复发情况。结果联合治疗组总有效率高于超声治疗组(P<0.05)。治疗期间,两组不良反应总发生率比较差异无统计学意义(P>0.05)。治疗1个月后,两组患者皮肤症状改善评分、炎性因子水平、DLQI评分较治疗前降低,且联合治疗组低于超声治疗组(P<0.05,P<0.01)。治疗后6个月、1年、2年,联合治疗组复发率低于超声治疗组(P<0.05,P<0.01)。结论采用中药熏洗联合高强度聚焦超声治疗湿热下注证外阴白色病变效果显著,可有效缓解患者临床症状和皮肤症状,降低炎性因子水平,提高患者生活质量,具有较好安全性。
Objective To investigate the clinical efficacy of traditional Chinese medicine(TCM)fumigation and washing and high-intensity focused ultrasound in the treatment of vulvar leukoplakia with damp-heat downward infusion syndrome.Methods From December 2021 to December 2022,the clinical data of patients with vulvar leukoplakia with damp-heat downward infusion syndrome were retrospectively analyzed.According to the treatment method,the patients were divided into ultrasound treatment group(n=97)and combination treatment group(n=103).The ultrasound treatment group was treated with high-intensity focused ultrasound,and the combination treatment group was treated with high-intensity focused ultrasound combined with TCM.The clinical effects and adverse reactions of the two groups were compared.The improvement of skin symptoms(vulvar pruritus,vulvar depigmentation,vaginal pain),inflammatory factor levels[hypersensitive C reactive protein,interleukin-6,procalcitonin]and quality of life[dermatology life quality index]were compared before treatment and at 1 month after treatment,and the recurrence after treatment in the two groups was recorded.Results The total effective rate in combination treatment group was higher than that in ultrasound treatment group(P<0.05).During treatment,there were no significant differences in the total incidence rates of adverse reactions between the two groups(P>0.05).At 1 month after treatment,the skin symptom improvement score,inflammatory factor levels and DLQI score in both groups were significantly declined compared with those before treatment,which were lower in combination treatment group than in ultrasound treatment group(P<0.05,P<0.01).At 6 months,1 year and 2 years after treatment,the recurrence rate was lower in combination treatment group than in ultrasound treatment group(P<0.05,P<0.01).Conclusion TCM fumigation and washing combined with highintensity focused ultrasound has significant efficacy in the treatment of vulvar leukoplakia with damp-heat downward infusion syndrome,and it can effectively relieve the clinical symptoms and skin symptoms of patients,reduce the levels of inflammatory factors and enhance the quality of life of patients,with good safety.
