摘要
总结张永臣教授运用“杂合以治”理念治疗膝骨关节炎的临床经验。张教授认为本病病位在筋骨肉,以肝肾脾亏虚为本,痰湿、瘀血痹阻为标,病性本虚标实。根据病机确立补益肝肾脾、化痰祛湿、化瘀止痛的治疗原则,宗“杂合以治”理念,以针刺为主,取“膝三针”(膝髌、犊鼻、内膝眼)、“二泉穴”(阳陵泉、阴陵泉)、“三太穴”(太冲、太溪、太白),施以导气法与龙虎交战针法,灵活组合运用艾灸、拔罐、中药外洗和耳穴埋针法以协同增效。附验案一则。
This article summarizes Professor ZHANG Yongchen’s clinical experience in treating knee osteoarthritis based on the theory of“integrated treatment”.Professor ZHANG believes that the disease lies in the muscles,tendons,and bones,with deficiency of liver,kidney,and spleen as the root cause,and phlegm-dampness,blood stasis,and obstruction of meridians as the symptoms.The nature of the disease is deficiency in origin and excess in superficiality.Based on the pathogenesis,the treatment principle is to tonify the liver,kidney,and spleen,eliminate phlegm and dampness,and dissolve blood stasis to relieve pain.Professor ZHANG adheres to“integrated treatment”concept and mainly uses acupuncture by taking the“Knee Tri-acupuncture”[Xibin,Dubi(ST35),Neixiyan(EX-LE4)]“,Two Spring Acupoints”[Yanglingquan(GB34),Yinlingquan(SP9)],and“Three Tai acupoints”[Taichong(LR3),Taixi(KI3),Taibai(SP3)]and applying the guiding-qi technique and dragon-tiger fighting needling technique.At the same time,a flexible combination of moxibustion,cupping,Chinese medicine external washing method and auricular acupuncture are used to synergize the effect.One test case is attached.
