摘要
膝骨关节炎(KOA)伴关节积液是临床常见的慢性骨关节疾病,以膝关节软骨退行性变、滑膜炎症为主要病理特征,膝关节疼痛、肿胀、功能障碍为核心表现,病程迁延反复,严重影响患者的生活质量,且女性多于男性。其病理核心为“滑膜炎症-积液-软骨损伤”的恶性循环,可归于中医学“膝痹”范畴,病机以“本虚标实”为核心,与肝肾亏虚、寒湿痹阻密切相关。火龙针针刺犊鼻、内膝眼、水分、血海、梁丘、阳陵泉、阴陵泉温通经络、散寒祛湿、调和气血、补益肝肾,针对疾病“本虚标实”的病机实现标本兼治,尤其适用于寒湿痹阻型KOA患者。火龙针依托针刺通络与热力持续温通的协同作用,可上调抗炎因子白细胞介素-10(IL-10)水平,调节神经酰胺代谢,改善局部微循环及血流灌注量,从而快速消退关节肿胀,缓解活动受限。
Knee osteoarthritis(KOA)with joint effusion is a common chronic osteoarticular disorder in clinical practice.Its primary pathological features include degenerative changes of knee articular cartilage and synovial inflammation,with core clinical manifestations of knee pain,swelling,and functional impairment.Due to its protracted and recurrent course,KOA significantly impairs patients’quality of life and shows a higher prevalence in women than in men.The underlying pathological mechanism involves a vicious cycle of“synovial inflammation-effusion-cartilage unblock”,which corresponds to the TCM syndrome of knee impediment.Its pathogenesis is characterized by root deficiency with tip excess,closely associated with liver-kidney deficiency and impediment due to cold-dampness.Huolong acupuncture applied at acupoints Dubi,Neixiyan,Shuifen,Xuehai,Liangqiu,Yanglingquan,and Yinlingquan can warm and dredge meridians,dispel cold and eliminate dampness,harmonize qi and blood,and tonify the liver and kidneys.This approach addresses both root and tip aspects of the disease according to its pathogenesis of root deficiency with tip excess,making it particularly suitable for patients with KOA of the cold-dampness impediment pattern.By synergistically combining needle-induced channel unblocking with sustained thermal stimulation,Huolong acupuncture can upregulate the anti-inflammatory cytokine interleukin-10(IL-10),modulate ceramide metabolism,improve local microcirculation and perfusion,thereby rapidly reducing joint swelling and alleviating activity limitations.
