摘要
腹泻型肠易激综合征(IBS-D)是一种常见的功能性肠道疾病,具有高发病率、高慢性化率、高复发率的特点,其发病机制尚未完全阐明。现代医学研究发现,胆汁酸-肠肝轴功能紊乱、VN/CAP抗炎通路功能障碍、肠道菌群-短链脂肪酸代谢轴失调与IBS-D发生和发展密切相关。从中医角度看,肝郁脾虚是IBS-D的核心病机,疏肝健脾作为IBS-D核心治法之一,其发挥作用的调控机制尚未系统阐述,现代医学通路研究缺乏系统整合。本文交叉分析中医理论与现代医学通路的关系,通过疏肝健脾理论整合IBS-D治疗的多通路协同调控机制,构建由胆汁酸-肠肝轴、VN/CAP抗炎通路、肠道菌群-短链脂肪酸代谢轴组成的参与IBS-D发生和发展及治疗的代谢-神经-免疫交互网络,以期为中医疏肝健脾理论现代化研究提供理论参考及生物学依据,为中医药多靶点治疗IBS-D提供理论依据,为中药精准化治疗IBS-D提供新思路。
Irritable bowel syndrome with diarrhea(IBS-D)is a common functional intestinal disease,has characteristics of high incidence rate,high chronic rate,and high recurrence rate,its pathogenesis has not been fully clarified.Modern medical research has found that dysfunction of bile acid-gut-liver axis,dysfunction of VN/CAP anti-inflammatory pathway,and dysregulation of intestinal flora-short chain fatty acid metabolism axis are closely related to occurrence and development of IBS-D.From the perspective of traditional Chinese medicine,stagnation of liver qi and spleen deficiency is core pathogenesis of IBS-D,dispersing stagnated liver qi for invigorating spleen as one of core treatment methods for IBS-D,regulatory mechanism of its function has not been systematically elucidated yet,lack of systematic integration in modern medical pathway research.This article cross analyzes relationship between traditional Chinese medicine theory and modern medical pathway,integrates multi-pathway collaborative regulation mechanism of IBS-D treatment through the theory of dispersing stagnated liver qi for invigorating spleen,constructs metabolism-nerve-immune interaction network involved in occurrence,development,and treatment of IBS-D,consisting of bile acid-gut-liver axis,VN/CAP anti-inflammatory pathway,and intestinal flora-short chain fatty acid metabolism axis,aim is to provide theoretical reference and biological basis for modernization research of traditional Chinese medicine theory of dispersing stagnated liver qi for invigorating spleen,provide theoretical basis for multi-target in the treatment of IBS-D with traditional Chinese medicine and pharmacy,provide new ideas for precision treatment of IBS-D with Chinese materia medica.
