摘要
目的:探讨脑动脉瘤性蛛网膜下腔出血后发生认知障碍的影响因素,据此制定干预措施。方法:回顾性分析医院收治的124例脑动脉瘤性蛛网膜下腔出血患者临床资料,根据其认知障碍发生情况分为发生组(23例)和未发生组(101例),构建Logistic回归模型分析脑动脉瘤性蛛网膜下腔出血后发生认知障碍的影响因素。结果:发生组患者的糖尿病、术前Hunt-HessⅢ-Ⅳ级、术后术区感染、年龄≥60岁占比均高于未发生组,差异均有统计学意义(P<0.05);Logistic回归分析显示,年龄≥60岁、糖尿病、术前Hunt-HessⅢ-Ⅳ级、术后术区感染脑动脉瘤性蛛网膜下腔出血后发生认知障碍的影响因素(OR>1,P<0.05)。结论:年龄≥60岁、糖尿病、术前Hunt-HessⅢ-Ⅳ级、术后术区感染脑动脉瘤性蛛网膜下腔出血后发生认知障碍的影响因素,临床可结合影响因素制定针对性干预措施,改善患者预后。
Objective:To investigate the influencing factors of cognitive impairment after cerebral aneurysmal subarachnoid hemorrhage(SAH),and to develop intervention measures accordingly.Methods:A retrospective analysis was conducted on the data of cerebral aneurysmal SAH admitted to the First People's Hospital of Jiujiang City.Based on the inclusion and exclusion criteria,124 clinical cases were selected.According to the occurrence of cognitive impairment,they were divided into the occurrence group(23 cases)and the nonoccurrence group(101 cases).A logistic regression model was constructed to examine the influencing factors of cognitive impairment after cerebral aneurysmal SAH.Results:The proportions of diabetes,preoperative Hunt-Hess grade Ⅲ-Ⅳ,postoperative surgical site infection,and age≥60 years in the occurrence group were higher than those in the non-occurrence group,with statistically significant differences(P<0.05).The constructed logistic regression model showed that age≥60 years,diabetes,preoperative Hunt-Hess grade Ⅲ-Ⅳ,and postoperative surgical site infection were influencing factors of cognitive impairment after cerebral aneurysmal SAH(OR>1,P<0.05).Conclusion:Age≥60 years,diabetes,preoperative Hunt-Hess grade Ⅲ-Ⅳ,and postoperative surgical site infection are influencing factors of cognitive impairment after cerebral aneurysmal SAH.In clinical practice,targeted intervention measures can be developed based on influencing factors to improve patient prognosis.
