摘要
目的:比较腹腔镜子宫肌瘤剔除术与传统开腹子宫肌瘤剔除术治疗子宫肌瘤的临床疗效及安全性。方法:采用回顾性研究方法,选取医院收治的98例子宫肌瘤患者,按随机数表法分为观察组(49例)和参照组(49例)。参照组行传统开腹子宫肌瘤剔除术,观察组行腹腔镜子宫肌瘤剔除术。比较两组患者的手术相关指标、术后不同时间点疼痛视觉模拟评分(VAS)、术后恢复情况及并发症发生率。结果:观察组患者手术时间、术中出血量、切口长度均优于参照组(P<0.05)。观察组患者术后6 h、12 h、18 h VAS评分均低于参照组,重复测量方差分析显示组别主效应、时间主效应及组别×时间交互效应差异均有统计学意义(P<0.05)。观察组患者肛门排气时间、下床活动时间、住院时间均短于参照组(P<0.05)。观察组患者术后并发症总发生率低于参照组(P<0.05)。结论:腹腔镜子宫肌瘤剔除术治疗子宫肌瘤具有手术创伤小、术中出血少、术后疼痛轻、恢复快及并发症发生率低等优势,临床应用价值显著,值得推广。
Objective:To compare the clinical efficacy and safety of laparoscopic myomectomy and traditional open myomectomy in the treatment of uterine fibroids.Methods:A retrospective study was conducted.98 patients with uterine fibroids admitted to our hospital were selected and divided into the observation group(n=49)and the reference group(n=49)according to the random number table method.The reference group underwent traditional open myomectomy,while the observation group underwent laparoscopic myomectomy.The surgical-related indicators of the two groups,pain visual analog scale(VAS)at different time points after surgery,postoperative recovery,and complication occurrence rate were compared.Results:The operation time,intraoperative blood loss,and incision length of the observation group were better than those of the reference group(P<O.05).The VAS scores of the observation group at 6 h,12 h,and 18 h after surgery were lower than those of the reference group,and repeated measurement variance analysis showed that the main effect of group,time,and the interaction effect of group×time were all statistically significant(P<O.05).The time of anal exhaust,time of getting out of bed,and hospital stay of the observation group were shorter than those of the reference group(P<0.05).The total incidence of postoperative complications in the observation group was lower in the reference group(P<O.05).Conclusion:laparoscopic myomectomy for uterine fibroids has advantages such as less surgical trauma,less intraoperative bleeding,less postoperative pain,faster recovery,and lower complication rate.Its clinical application value is significant and is worthy of promotion.
