摘要
本文基于“脾气散精”理论探讨江氏运脾法在女童中枢性性早熟防治中的应用。通过论述“脾气散精”在儿童生长发育中的作用,为女童中枢性性早熟“脾不散精”病理理论奠定基础。提出脾虚失运,精微不布,痰湿壅滞;肝失疏泄,土壅木郁,气火扰任;脾肾失赞,肾阴亏虚,相火妄动三大关键病机。治疗以江氏运脾法为理论指导,强调运脾土而散精微,消痰脂以安天癸,疏肝木以调冲任,滋肾水而降相火,为中医药防治女童中枢性性早熟提供新的思路与方法。
Based on the theory of“spleen-qi essence distribution”,this study elucidates the application of Jiang Yuren’s spleenregulating method in the management of central precocious puberty(CPP)in girls.By demonstrating the role of spleen-qi essence distribution in child growth and development,this study establishes the pathological basis of the spleen’s failure to distribute essence in CPP in girls.This study proposes three key pathodynamic processes,including spleen-qi dysfunction resulting in impaired essence distribution and phlegm-damp stagnation;compromised liver qi dredging due to spleen-qi stagnation and liver-qi depression,disturbing the Chongren system;disrupted spleen-kidney synergy leading to kidney yin deficiency and hyperactivity of ministerial fire.The therapeutic strategy,guided by Jiang Yuren’s“spleen-regulating method”,emphasizes regulating spleen-qi to distribute essence,resolving phlegm-lipid accumulation to stabilize Tiangui,soothing liver-qi to modulate Chongren system,and nourishing kidney-yin to restrain ministerial fire.This approach provides a new theoretical framework and clinical methodology in Chinese medicine for the management of CPP in girls.
