摘要
目的:探讨牛膝温经汤治疗寒湿痹阻型膝骨关节炎的临床疗效。方法:选取80例寒湿痹阻型膝骨关节炎患者,通过随机数字表法分为观察组和对照组(各40例)。对照组口服塞来昔布胶囊,0.2 g/次,2次/d;观察组口服牛膝温经汤,1剂/d,分2次温服。4周后观察两组患者VAS评分、中医证候评分、WOMAC评分、肌骨超声下滑膜评价及血清炎症因子IL-1β、TNF-α治疗后的变化。结果:两组患者各观察指标治疗前比较,差异无统计学意义(P>0.05);治疗后各观察指标评分较其治疗前明显降低,差异有统计学意义(P<0.01);治疗后与对照组比较,观察组患者中医证候评分及WOMAC评分下降,差异有统计学意义(P<0.01)。结论:牛膝温经汤治疗寒湿痹阻型膝骨关节炎疗效满意,其机制可能与炎症因子IL-1β、TNF-α的降低有关。
Objective:To discuss the clinical efficacy of Niuxi Wenjing decoction in the treatment of knee osteoarthritis(KOA)with cold-dampness obstruction syndrome.Methods:Eighty patients with KOA and cold-dampness obstruction were randomly divided into observation group and control group with 40 cases in each group by using random number table method.The control group received oral celecoxib capsules,0.2 g each times,twice daily,whereas the observation group was given oral Niuxi Wenjing decoction,1 dose per day,taken warmly twice a day.Both treatments were given for 4 weeks.The changes in VAS scores,TCM symptom scores,WOMAC scores,musculoskeletal ultrasound assessments,and serum levels of IL-1β and TNF-α were observed before and after treatment.Results:No statistically significant differences were observed in any of observation indicators between the two groups before treatment(P>0.05).After treatment,significant decreases were found in all observation indicators within each group compared to their respective baseline levels(P<0.01).Compared with the control group,the observation group showed lower scores after treatment for both TCM syndrome assessment and WOMAC scores(P<0.01).Conclusion:Niuxi Wenjing decoction shows significant efficacy in the treatment of KOA with cold-dampness obstruction.The mechanism may be associated with the reduction of inflammatory factors(IL-1β,TNF-α).
