咖啡因联合无创呼吸支持在早产儿呼吸暂停中的应用

摘要 目的探讨咖啡因联合无创呼吸支持在早产儿呼吸暂停中的应用效果。方法选取2020年1月至2024年12月南京医科大学附属江宁医院新生儿科收治的呼吸暂停早产儿88例为研究对象,采用随机数字表法分为两组。观察组(n=44)使用咖啡因联合无创呼吸支持治疗,对照组(n=44)应用无创呼吸支持治疗,比较两组呼吸暂停情况、血气指标、临床指标。结果观察组72 h内呼吸暂停次数少于对照组,恢复正常呼吸节律时间早于对照组(P<0.05)。治疗后,观察组动脉血氧分压、动脉血氧饱和度、pH高于对照组,动脉血二氧化碳分压低于对照组(P<0.05)。观察组无创呼吸支持时间、总氧疗时间短于对照组,完全经口喂养时间早于对照组(P<0.05)。结论咖啡因联合无创呼吸支持在早产儿呼吸暂停中的应用效果较好,可减少呼吸暂停发作次数,纠正血气异常,缩短呼吸支持时间,促进康复。 Objective To explore the application effect of Caffeine combined with noninvasive respiratory support in premature infants with apnea.Methods A total of 88 premature infants with apnea in Department of Neonatology,the Affiliated Jiangning Hospital of Nanjing Medical University from January 2020 to December 2024 were selected as the research objects and were divided into two groups by the random number table method.The observation group(n=44)was treated with Caffeine combined with noninvasive respiratory support,and the control group(n=44)was treated with noninvasive respiratory support.The conditions of apnea,blood gas indicators and clinical indicators of the two groups were compared.Results The number of apnea within 72 hours in the observation group was fewer than that in the control group,and the time to restore normal respiratory rhythm was earlier than that in the control group(P<0.05).After treatment,the arterial partial pressure of oxygen,arterial oxygen saturation,and pH in the observation group were higher than those in the control group,while the arterial partial pressure of carbon dioxide was lower than that in the control group(P<0.05).The duration of non-invasive respiratory support and total oxygen therapy in the observation group was shorter than that in the control group,and the time for complete oral feeding was earlier than that in the control group(P<0.05).Conclusion The application of Caffeine combined with noninvasive respiratory support in premature infants with apnea shows a better effect,which can reduce the frequency of apnea,correct blood gas abnormalities,shorten the duration of respiratory support,and promote recovery.
