摘要
青光眼是临床常见的严重损坏视觉功能的不可逆性致盲眼病。西医治疗以药物、激光或手术等方式降眼压为主,但存在一定的风险和复发率。中医学可以突破传统西医治疗的局限,在青光眼的治疗中发挥独特优势。本文基于“血水同治”理论,对青光眼及其围手术期的病因病机进行了详细探究,并且阐明了“血水同治”治疗青光眼及其围手术期的可行性,认为该病病机主要责之于肝、肾、三焦功能失调,气血运行不畅,瘀血积聚,津液输布失常,水湿内停,致目系失养,玄府闭塞,神水瘀积。瘀血与水湿互为因果,相互胶结而发病,故治疗上当以活血利水为治则,灵活辨证,依据不同阶段,加用益气养阴、益气养血药,以达疏通眼部瘀滞之脉络,加快神水之循环之效,为青光眼及其围手术期的临床治疗提供依据。
Glaucoma is an irreversible blinding eye disease that severely damages visual function.Western Medicine primarily relies on intraocular pressure(IOP)reduction through medications,laser,or surgery,yet these approaches carry certain risks and recurrence rate.Traditional Chinese Medicine(TCM)can overcome the limitations of conventional Western Medicine and offer unique advantages in glaucoma management.Based on the theory of simultaneous treatment of blood and water,this paper explores the etiology and pathogenesis of glaucoma and its perioperative period in detail,and clarifies the feasibility of applying this theory.The pathogenesis is primarily attributed to dysfunction of liver,kidney,and Sanjiao,leading to impeded circulation of Qi and blood,accumulation of static blood,impaired distribution of body fluids,internal retention of dampness,malnutrition of the ocular system,obstruction of Xuanfu,and stasis of the aqueous humor(Shenshui).Static blood and dampness interact as both cause and effect,intertwining to precipitate the disease.Therefore,the treatment principle should be activating blood and promoting water discharge,with flexible pattern differentiation.According to different stages,medicinals for boosting Qi and nourishing Yin,or boosting Qi and nourishing blood,are added to unblock stagnant collaterals in the eye and accelerate the circulation of the aqueous humor(Shenshui).This provides a basis for the clinical treatment of glaucoma and its perioperative period.
