摘要
目的探讨结直肠癌根治术患者健康素养的发展轨迹,并分析其影响因素,为制定针对性的干预措施提供理论依据。方法选取2021年11月-2024年11月在浙江大学医学院附属邵逸夫医院接受结直肠癌根治术的256患者为研究对象。采用一般资料调查表、慢性病病人健康素养量表、癌症自我管理效能感量表和社会支持评定量表对其进行调查,分析其健康素养发展轨迹的影响因素。结果结直肠癌根治术患者健康素养发展轨迹可分为4个类别:中素养-改善型(39.5%)、低素养-维持型(15.2%)、高素养-较好型(20.3%)、低素养-改善型(25.0%)。Logistic回归分析结果显示,文化程度、家庭人均月收入、居住地、术后并发症、术后延续护理、自我效能感、社会支持水平是结直肠癌根治术患者健康素养发展轨迹的独立影响因素(均P<0.05)。结论结直肠癌根治术患者的健康素养存在群体异质性,医护人员应依据患者健康素养发展轨迹特征及其影响因素制定针对性的干预措施,确保患者术后健康素养逐步改善,达到较好的标准。
Objective To explore the development of health literacy in patients undergoing radical resection of rectal cancer,and analyze its influencing factors,so as to provide a theoretical basis for future targeted intervention.Methods A total of 256 patients who underwent radical resection of colorectal cancer at Sir Run Run Shaw Hospital Affiliated to School of Medicine,Zhejiang University,between November 2021 and November 2024 were selected as the research subjects.They were surveyed using the general information questionnaire,the health literacy management scale,the strategies used by people to promote health and the social support rating scale.Moreover,the influencing factors of the differences in development trajectories were analyzed.Results The health literacy development of patients undergoing radical resection of colorectal cancer can be classified into four categories:medium literacy-improvement type(39.5%),low literacy-maintenance type(15.2%),high literacy-good type(20.3%),and low literacy-improvement type(25.0%).The Logistic regression analysis showed educational level,per capita monthly income of the family,place of abode,postoperative complications,continuous nursing,self-efficacy,and social support level were independent influencing factors for the differences in the development trajectory of health literacy among such patients(all P<0.05).Conclusion There is group heterogeneity in the development of health literacy among patients after radical resection of colorectal cancer.Therefore,medical staff should formulate corresponding intervention measures to improve their health literacy.
