经颅直流电刺激联合醒脑开窍针刺法治疗卒中后认知障碍的临床疗效

Clinical Efficacy of Transcranial Direct Current Stimulation Combined with Acupuncture for Awakening the Brain and Opening the Orifices in the Treatment of Cognitive Impairment After Stroke

摘要 目的:探讨经颅直流电刺激(tDCS)联合醒脑开窍针刺法治疗卒中后认知障碍的临床疗效。方法:采用随机数字表法将150例卒中后认知障碍患者随机分为联合组、针刺组、tDCS组,每组各50例。所有患者均接受常规药物治疗(盐酸多奈哌齐5 mg/d+盐酸美金刚5 mg/d)及认知功能训练。联合组在常规治疗基础上加用tDCS联合醒脑开窍针刺法;针刺组加用醒脑开窍针刺法;tDCS组加用tDCS。治疗28 d后采用MMSE、MoCA、Rivermead行为记忆功能、FIM及ADL评分评估三组疗效。结果:治疗前三组各项评分差异无统计学意义(P>0.05),治疗后联合组MMSE、MoCA、Rivermead行为记忆功能、FIM及ADL评分显著优于针刺组和tDCS组(P<0.01)。结论:tDCS联合醒脑开窍针刺法可显著改善卒中后认知功能,疗效优于单纯使用醒脑开窍针刺法和单独使用tDCS。 Objective To investigate the clinical efficacy of transcranial direct current stimulation(tDCS)combined with acupuncture for awakening the brain and opening the orifices(XNKQ)in the treatment of cognitive impairment after stroke.Methods A total of 150 patients diagnosed with cognitive impairment after stroke were assigned to three groups using a random number table method:combination group,acupuncture group,and tDCS group,with 50 cases in each group.All participants received standard pharmacological treatment(donepezil hydrochloride 5 mg/d+memantine hydrochloride 5 mg/d)and underwent cognitive function training.In addition to basic treatment,the combination group received tDCS combined with XNKQ;the acupuncture group was treated with XNKQ;the tDCS group was given tDCS alone.The therapeutic effects across the three groups were evaluated after 28 d of intervention using the Mini-Mental State Examination(MMSE),Montreal Cognitive Assessment(MoCA),Rivermead Behavioral Memory Test,Functional Independence Measure(FIM),and Activities of Daily Living(ADL)scores.Results No differences were observed in the scores among the three groups before treatment(P>0.05).After treatment,the combination group demonstrated more improvements in MMSE,MoCA,Rivermead behavioral memory function,FIM,and ADL scores than both acupuncture group and tDCS group(P<0.01).Conclusion The combination of tDCS and XNKQ significantly enhances cognitive function in post-stroke patients and has superior efficacy compared with XNKQ or tDCS alone.
