摘要
目的:验证阳和解凝膏治疗糖尿病足溃疡阴证的临床疗效,并揭示其促进创面愈合的潜在机制。方法:将96名糖尿病足溃疡患者随机分为对照组和治疗组,各48例。对照组采用西医常规治疗,治疗组在常规治疗基础上加用阳和解凝膏外敷。观察两组患者治疗前后创面愈合及疼痛和跛行情况,以及创面组织中白介素-6(IL-6)、白介素-1β(IL-1β)、超氧化物歧化酶(SOD)、丙二醛(MDA)、糖基化终末产物(AGEs)、血管内皮生长因子(VEGF)指标变化。结果:治疗4周后,治疗组溃疡面积小于对照组;视觉模拟评分(VAS)和溃疡组织中的IL-6、IL-1β、MDA、AGEs指标水平均低于对照组,SOD、VEGF水平高于对照组(P<0.05)。结论:阳和解凝膏可加速糖尿病足溃疡阴证创面愈合,其作用机制可能与通过降低炎症因子表达、改善溃疡组织局部氧化应激水平及高糖微环境相关。
Objective:To evaluate the clinical efficacy of Yanghe Jiening(YHJN)ointment in the treatment of diabetic foot ulcers with Yin-syndrome pattern and to explore its potential mechanisms in promoting wound healing.Methods:A total of 96 patients with diabetic foot ulcers were randomly divided into a control group and a treatment group with 48 cases each.The control group received standard western medical care alone,while the treatment group received topical application of YHJN besides the routine treatment.Changes in wound healing,pain,and claudication were observed before and after treatment in both groups,and the alterations in the levels of interleukin-6(IL-6),interleukin-1β(IL-1β),superoxide dismutase(SOD),malondialdehyde(MDA),advanced glycation end products(AGEs),and vascular endothelial growth factor(VEGF)in wound tissues were also observed.Results:After 4 weeks of treatment,the ulcer area was smaller,the visual analogue scale(VAS)score and levels of IL-6,IL-1β,MDA,and AGEs in ulcer tissue were lower,while SOD and VEGF levels were higher in the treatment group than the control group(P<0.05).Conclusion:YHJN can accelerate wound healing in diabetic foot ulcers with Yin-syndrome pattern.Its mechanism may be related to the reduction of inflammatory factor expression and improvement of local oxidative stress status and the hyperglycemic microenvironment in ulcer tissue.
