摘要
目的:探讨血小板减少症患者ST6β-半乳糖胺α2,6-唾液酸转移酶1(ST6Gal-1)水平和血小板CD36去唾液酸化修饰的变化及相关性。方法:纳入本院2023年2月-2024年10月收治的302例血小板减少症患者进行回顾性研究。根据流式细胞术检测的血小板膜糖蛋白CD36表达情况将患者分为血小板CD36阳性组(n=292)和血小板CD36阴性组(n=10)。检测两组患者血小板功能、血小板表面唾液酸化修饰[蓖麻凝集素1(RCA-1)、鸡冠刺桐凝集素(ECL)、小麦胚芽凝集素(WGA)]、血小板中ST6Gal-1水平。通过Pearson相关性检验分析不同CD36表达情况的血小板减少症患者血小板表面唾液酸化修饰与ST6Gal-1水平的相关性。结果:血小板CD36阴性组患者PLT、血小板功能正常比例、RCA-1、ECL、WGA平均荧光强度及ST6Gal-1 mRNA相对表达量低于血小板CD36阳性组,疾病严重程度更重,血小板功能低下比例更高(P<0.05);在所有血小板减少症患者中,血小板ST6Gal-1 mRNA相对表达量与血小板RCA-1、ECL、WGA平均荧光强度均呈弱正相关性(r=0.384、0.436、0.328,P<0.05)。在血小板CD36阴性组患者中,血小板ST6Gal-1 mRNA相对表达量与血小板RCA-1、ECL、WGA平均荧光强度均呈强正相关性(r=0.956、0.937、0.897,P<0.05)。在血小板CD36阳性组患者中,血小板ST6Gal-1 mRNA相对表达量与血小板ECL、RCA-1、WGA平均荧光强度呈弱正相关性(r=0.329、0.385、0.278,P<0.01)。结论:血小板CD36阴性的血小板减少症患者ST6Gal-1、唾液酸水平低于血小板CD36阳性患者,且ST6Gal-1与血小板去唾液酸化呈正相关性,检测血小板ST6Gal-1水平可能为血小板减少症未来辅助诊断及病情预测提供参考。
Objective:To investigate the changes and correlation between the level of ST6β-galactosamineα2,6-sialyltransferase 1(ST6Gal-1)and the desialylation modification of platelet CD36.Method:A retrospective study was conducted on 302 patients with thrombocytopenia admitted to our hospital from February 2023 to October 2024.According to the expression of platelet membrane glycoprotein CD36 detected by flow cytometry,the patients were divided into platelet CD36-positive group(n=292)and platelet CD36-negative group(n=10).The following indicators of the patients were detected:platelet function,sialylation modification on the platelet surface[ricinus communis agglutinin I(RCA-1),erythrina cristagalli lectin(ECL),wheat germ agglutinin(WGA)],and the level of ST6Gal-1 in platelets.Pearson correlation test was used to analyze the correlation between the platelet surface sialylation level and ST6Gal-1 level in thrombocytopenia patients with different CD36 expression statuses.Results:Compared with the CD36-positive platelet group,the CD36-negative platelet group showed the following characteristics:lower platelet count(PLT),lower proportion of normal platelet function,lower mean fluorescence intensity(MFI)of RCA-1,ECL,and WGA,and lower relative expression level of ST6Gal-1 mRNA.Meanwhile,the disease severity was more significant,and the proportion of decreased platelet function was higher.All these differences were statistically significant(P<0.05).Among all patients with thrombocytopenia,the relative expression level of platelet ST6Gal-1 mRNA showed a weak positive correlation with the MFI of platelet RCA-1,ECL,and WGA(r=0.384,0.436,0.328,P<0.05).In patients with CD36-negative platelets,the relative expression level of platelet ST6Gal-1 mRNA exhibited a strong positive correlation with the MFI of platelet RCA-1,ECL,and WGA(r=0.956,0.937,0.897,P<0.05).In patients with CD36-positive platelets,the relative expression level of platelet ST6Gal-1 mRNA showed a weak positive correlation with the MFI of platelet ECL and RCA-1 corresponding to ECL,RCA-1,and WGA(r=0.329,0.385,0.278,respectively,P<0.01).Conclusion:The levels of ST6Gal-1 and sialic acid in thrombocytopenia patients with CD36-negative platelets are lower than those in patients with CD36-positive platelets,and ST6Gal-1 shows a positive correlation with platelet desialylation.Detecting the platelet ST6Gal-1 level may provide a certain basis for auxiliary diagnosis and prediction of thrombocytopenia in the future.
