摘要
目的探讨柴芩温胆汤对特发性肺纤维化(IPF)患者的治疗效果及对机体高凝状态和抗氧化功能的调节作用。方法选取2023年1月至2024年9月于该院就诊的IPF患者80例为研究对象,随机分为对照组和观察组,每组各40例。对照组服用吡非尼酮,观察组予以吡非尼酮+柴芩温胆汤口服。比较两组治疗3个月后的疗效差异,中医证候积分,肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占用力肺活量百分比(FEV_(1)/FVC)、肺一氧化碳弥散量(DLCO)],凝血指标[纤维蛋白原(FIB)、D-二聚体(D-D)]水平变化。采用酶联免疫吸附试验检测血清纤维化指标[透明质酸(HA)、层黏蛋白(LN)、Ⅲ型胶原(Ⅲ-C)、Ⅳ型胶原(Ⅳ-C)]及血浆氧化应激指标[超氧化物歧化酶(SOD)、脂质过氧化物(MDA)、谷胱甘肽过氧化物酶(GSH-Px)]水平变化。结果治疗前,对照组和观察组上述指标比较,差异均无统计学意义(P>0.05)。治疗3个月后,两组中医症候积分及FIB、D-D、HA、LN、Ⅲ-C、IV-C、MDA水平均降低(P<0.05),且观察组低于对照组(P<0.05),FVC、FEV_(1)、FEV_(1)/FVC、DLCO、SOD、GSH-Px水平升高(P<0.05),且观察组高于对照组(P<0.05)。结论柴芩温胆汤结合吡非尼酮治疗IPF具有较好疗效,可显著提高患者肺功能,改善纤维化状态,缓解机体氧化应激反应和血液高凝状态,进一步提升吡非尼酮的治疗效果。
Objective To investigate the therapeutic effect of Chaiqin Wendan decoction on patients with idiopathic pulmonary fibrosis(IPF)and its regulatory effect on hypercoagulable state and antioxidant function.Methods A total of 80 patients with IPF who were treated in this hospital from January 2023 to September 2024 were selected as the research objects,and were randomly divided into a control group and an observation group,with 40 cases in each group.The control group was given pirfenidone,and the observation group was given pirfenidone+Chaiqin Wendan decoction.The two groups were compared in terms of therapeutic effect,Traditional Chinese Medicine syndrome score,pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in first second(FEV_(1)),forced expiratory volume in first second to forced vital capacity(FEV_(1)/FVC),lung carbon monoxide diffusion capacity(DLCO)],and coagulation indexes[fibrinogen(FIB),D-dimer(D-D)]after 3 months of treatment.Enzyme-linked immunosorbent assay was used to detect the changes in serum fibrosis markers[hyaluronic acid(HA),laminin(LN),typeⅢcollagen(Ⅲ-C),typeⅣcollagen(Ⅳ-C)]and plasma oxidative stress indicators[superoxide dismutase(SOD),lipid peroxide(MDA),glutathione peroxidase(GSH-Px)].Results There were no significant differences in the above indexes between the two groups before treatment(P>0.05).After 3 months of treatment,the Traditional Chinese Medicine syndrome scores and levels of FIB,D-D,HA,LN,Ⅲ-C,IV-C and MDA in the two groups were decreased(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).The levels of FVC,FEV_(1),FEV_(1)/FVC,DLCO,SOD and GSH-Px were increased(P<0.05),and those in the observation group were higher than those in the control group(P<0.05).Conclusion Chaiqin Wendan decoction combined with pirfenidone in the treatment of IPF has a good curative effect,which can significantly improve the lung function of patients,improve fibrosis,alleviate oxidative stress and blood hypercoagulable state,and further enhance the therapeutic effect of pirfenidone.
