摘要
目的探讨复方黄柏液联合头孢呋辛钠对痔切除术后患者切口愈合及炎症水平的影响。方法采用前瞻性随机对照研究设计,选取2023年10月至2025年3月衡水市第四人民医院收治的312例行痔切除术患者,按随机数字表法将其分为观察组与对照组,各156例。两组术后均予常规对症及头孢呋辛钠抗感染治疗,观察组加用复方黄柏液涂剂坐浴。治疗2周后,比较两组临床疗效、切口愈合时间、创面水肿、疼痛视觉模拟评分(VAS)、血清炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)]变化情况及并发症发生情况。结果疾病控制率、水肿控制率观察组均高于对照组[90.39%(141/156)vs 76.93%(120/156)、[79.49%(124/156)vs 46.79%(73/156)],差异有统计学意义(χ^(2)=10.337、35.820,P<0.001)。观察组切口愈合时间短于对照组[(16.19±3.52)d vs(20.33±4.47)d,t=9.088,P<0.001)]。治疗后第3天、第7天,观察组VAS评分低于对照组(t=4.631、14.144,P<0.001);重复测量方差分析显示,VAS评分组间、时间及交互效应均有统计学意义(F_(组间)=15.632,P<0.001;F_(时间)=125.367,P<0.001;F_(交互)=85.214,P<0.001)。观察组TNF-α[(3.12±0.41)ng/mL vs(4.05±0.65)ng/mL]、IL-6[(12.49±3.63)ng/mL vs(15.63±4.67)ng/mL]水平低于对照组(t=15.114、6.630,P<0.001)。观察组并发症发生率低于对照组[8.33%(13/156)vs 18.59%(29/156),χ^(2)=7.043,P<0.05]。结论在痔切除术后常规抗生素治疗基础上联合复方黄柏液,可促进切口愈合,减轻疼痛、水肿及炎症反应,降低并发症发生率,安全有效。
Objective To investigate the effects of Compound Huangbai Liquid combined with cefuroxime sodium on wound healing and inflammatory levels in patients after hemorrhoidectomy.Methods A prospective randomized controlled study design was adopted.A total of 312 patients who underwent hemorrhoidectomy at the No.4 People’s Hospital of Hengshui from October 2023 to March 2025 were selected and randomly divided into the observation group and the control group using a random number table method,with 156 cases in each group.Both groups received routine symptomatic treatment and anti-infective therapy with cefuroxime sodium postoperatively.The observation group additionally received Compound Huangbai Liquid lotion for sitz baths.After 2 weeks of treatment,clinical efficacy,wound healing time,wound edema,Visual Analogue Scale(VAS)pain scores,serum levels of inflammatory factors(tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)),and the incidence of complications were compared between the two groups.Results The observation group showed higher disease control rate(90.39%(141/156)vs 76.93%(120/156))and edema control rate(79.49%(124/156)vs 6.79%(73/156))compared to the control group(χ^(2)=10.337,35.820,both P<0.001).The wound healing time was significantly shorter in the observation group than in the control group((16.19±3.52)days vs(20.33±4.47)days,t=9.088,P<0.001).On day 3 and day 7 after treatment,VAS scores were lower in the observation group than those in the control group(t=4.631,14.144,both P<0.001).Repeated measures analysis of variance showed statistically significant effects in VAS scores across groups,over time,and in interaction effects.(F_(group)=15.632,P<0.001;F_(time)=125.367,P<0.001;F_(interaction)=85.214,P<0.001).The observation group also exhibited significantly lower levels of TNF-α((3.12±0.41)ng/mL vs(4.05±0.65)ng/mL,t=15.114,P<0.001)and IL-6((12.49±3.63)ng/mL vs(15.63±4.67)ng/mL,t=6.630,P<0.001)compared to the control group.The incidence of complications in the observation group was significantly lower than that in the control group(8.33%(13/156)vs 18.59%(29/156),χ^(2)=7.043,P<0.05).Conclusion The addition of Compound Huangbai Liquid to conventional antibiotic therapy after hemorrhoidectomy can significantly promote wound healing,alleviate pain and edema,reduce inflammatory response,lower complications,with a good safety and efficacy profile.
