摘要
目的:探讨个体化定制音乐治疗对主观性耳鸣患者的临床疗效,并分析患者人口学特征、耳鸣参数及心理状态对疗效的影响。方法:纳入2020年6月至2022年5月温州医科大学附属第一医院200例主观性耳鸣患者,采用专利软件生成基于听觉阈值、耳鸣特征及心理评估的个性化定制音乐治疗方案,治疗时长为每日2 h,持续6个月。通过耳鸣致残量表(THI)、视觉模拟量表(VAS)、耳鸣评价量表(TEQ)、残余抑制(RI)和耳鸣响度变化评估疗效,分析年龄、性别、病程等人口学特征与耳鸣类型、频率、响度等因素对疗效的影响。结果:总体治疗有效率为66%,治疗后THI、VAS和TEQ评分显著降低(均P<0.05),平均RI时间为63.6 s,耳鸣响度从44.67 dB降至38.71 dB(P<0.05)。耳鸣阈上响度(r=0.46,P<0.05)是疗效的关键影响因素,心理状态以及性别、年龄等人口学因素对疗效均无显著影响(均P>0.05)。结论:个体化定制音乐治疗能够降低主观性耳鸣患者的THI、VAS和TEQ评分,可有效缓解耳鸣症状产生的困扰,且耳鸣响度调控是影响疗效的核心因素。
Objective:To evaluate the clinical efficacy of personalized music therapy(PMT)for patients with subjective tinnitus and analyze the impact of demographic characteristics,tinnitus parameters and psychological state on the efficacy.Methods:A total of 200 patients with subjective tinnitus were recruited from the First Affiliated Hospital of Wenzhou Medical University(June 2020-May 2022).A proprietary softwaregenerated PMT protocol was used(2 hours/day for 6 months)incorporating auditory threshold,tinnitus characteristics and psychological assessment.The efficacy was evaluated by the tinnitus handicap inventory(THI),visual analogue scale(VAS),tinnitus evaluation questionnaire(TEQ),residual inhibition Time(RI)and tinnitus loudness change.The impact of demographic variables(age,sex,disease duration)and tinnitus-related factors(type,frequency,loudness)on treatment outcomes was further analyzed.Results:The overall effective rate was 66%.THI,VAS,and TEQ scores significantly decreased after treatment(all P<0.05).The mean residual inhibition(RI)duration was 63.6 seconds,and tinnitus loudness decreased from 44.67 dB to 38.71 dB(P<0.05).Suprathreshold tinnitus loudness(r=0.46,P<0.05)was identified as a key predictive factor for treatment response.Psychological status(r=0.38,P>0.05)showed no significant correlation with therapeutic efficacy,and neither did demographic variables such as age and sex(all P>0.05).Conclusion:Individualized customized music therapy effectively reduces THI,VAS,and TEQ scores in patients with subjective tinnitus and alleviates tinnitus-related distress.Tinnitus loudness regulation is the core determinant of therapeutic efficacy.
