摘要
目的:探讨3.0T磁共振T2*mapping成像分区定量评估膝骨性关节炎(knee osteoarthritis,KOA)软骨损伤的应用价值。膝骨性关节炎是一种常见的退行性疾病,以关节软骨变性、软骨下骨重塑和骨赘形成为特征,严重影响患者的生活质量,早期诊断和干预对于延缓疾病进展至关重要。方法:回顾性分析本院80例膝骨性关节炎患者的X线、常规MRI和T2*mapping图像资料,根据患者的膝骨性关节炎严重程度分为早期KOA组(n=54)、晚期KOA组(n=26),分析比较两组患者膝关节各亚区软骨的厚度、T2*值及WORMS软骨缺损评分。结果:晚期KOA组患者髌骨外侧、股骨外侧前区、股骨及胫骨内侧前区、中央区软骨厚度均低于早期KOA组(P<0.05)。晚期KOA组中有11个亚区T2*值均升高(P<0.05)。另外,各软骨亚区浅层软骨的T2*值均显著高于同亚区深层软骨(P<0.001)。KOA患者的髌骨、股骨内侧、股骨外侧、胫骨内侧及胫骨外侧软骨亚区T2*值与WORMS软骨缺损评分呈轻度至强相关(r_(s)=0.401、0.267、0.500、0.624、0.582,P<0.05)。膝关节软骨亚区的厚度和T2*值评估膝骨性关节炎严重程度的AUC分别为0.745、0.815,两项联合评估的AUC为0.863。结论:膝关节T2*mapping软骨成像多区测量可更加准确地识别损伤部位,并进一步定量评估软骨损伤情况,为膝骨性关节炎的早期诊断及针对性干预提供了较为可靠的依据。
Objective:To evaluate the application value of 3.0T magnetic resonance T2*mapping imaging in multi-subregion assessing of cartilage damage in knee osteoarthritis(KOA).KOA is a common degenerative disease characterized by cartilage degeneration,subchondral bone remodeling,and osteophyte formation,significantly affecting patients'quality of life.Early diagnosis and intervention are crucial for delaying disease progression.Methods:A retrospective analysis was conducted on X-ray,conventional MRI,and T2*mapping image data from 80 patients with KOA in our hospital.Patients were classified according to the severity of their knee osteoarthritis into early KOA(n=54)and advanced KOA(n=26).We analyzed the thickness and T2*values of cartilage,and WORMS cartilage defect scores in various subregions of the knee joint between the two groups.Results:In the analysis of cartilage subzone thickness,the cartilage thickness of the lateral patella,lateral anterior femur,medial anterior femur and tibia,and central zone of the patients in the advanced KOA were lower than those in the early KOA(P<0.05).In the analysis of T2*values of cartilage subregions,11 subregions had elevated T2*values in the advanced KOA group(P<0.05).In addition,the T2*values of the superficial cartilage in each cartilage subregion were significantly higher than those of the deep cartilage in the same subregion(P<0.001).The T2*values of the patellar,medial femur,lateral femur,medial tibia,and lateral tibia cartilage subregions of the patients with KOA showed a mild to strong correlation with WORMS cartilage defect scores(r_(s)=0.401,0.267,0.500,0.624,and 0.582,P<0.05).The AUC of knee cartilage thickness and T2*values of the subregion on the severity of knee osteoarthritis were 0.745 and 0.815,and the AUC of the combined assessment was 0.863.Conclusion:T2*mapping cartilage imaging multi-zone measurements of the knee joint can more accurately identify the site of injury and further quantitatively assess the cartilage damage,providing a more reliable basis for early diagnosis of knee osteoarthritis and targeted intervention.
