2023年秋季大连市儿童肺炎支原体感染流行特征分析

摘要 目的:分析2023年秋季大连市0~14岁儿童肺炎支原体感染的流行病学特征及临床表现,为制定有效的防控措施和优化治疗方案提供参考。方法:选取2023年9—10月大连市部分地区0~14岁儿童492人,调查呼吸道感染儿童中肺炎支原体感染率、年龄与性别分布特征、临床表现、就诊及服药情况等。结果:492名儿童中有197人发生呼吸道感染,发病率为40.04%。在就诊的93名呼吸道感染患儿中,18.28%(17例)为肺炎支原体感染。0~14岁儿童各年龄组均可感染肺炎支原体,其中4~7岁组的感染率最高,为26.09%。肺炎支原体感染患儿以高热(>38℃)为主要临床表现,平均病程为11天,较呼吸道感染的平均病程更长(P<0.05)。就诊的17例肺炎支原体患儿中,有13例(76.47%)选择三级医院诊治,有15例(88.24%)使用了大环内酯类抗菌药物。结论:在0~14岁儿童中,4~7岁儿童为肺炎支原体易感人群。肺炎支原体感染患儿病程较长,且不规范使用大环内酯类抗菌药物现象广泛。 OBJECTIVE To analyze the epidemiological characteristics and clinical manifestations of Mycoplasma pneumoniae(MP)infection in children aged 0~14 in Dalian in the autumn of 2023,and to provide reference for developing effective prevention and control measures and the optimization of treatment strategies.METHODS 492 Children aged O~14 in some areas of Dalian from September to October 2023 were selected as the survey subjects to study on the infection rate,age and gender characteristics,clinical manifestations,distribution of medical visits and medication use of Mycoplasma pneumoniae in children with respiratory tract infections.RESULTS The infection number among 492 children was 197(40.04%).18.28%of the 93 children diagnosed with respiratory infections were infected with Mycoplasma pneumoniae.Children aged O~14 can be infected with Mycoplasma pneumoniae in all age groups,with the highest infection rate of 26.09%in the 4~7 age group.The main clinical manifestation of Mycoplasma pneumoniae infection in children was high fever(>38 C),with an average course of 11 days longer than respiratory infections(P<0.05).13(76.47%)of IT children with Mycoplasma pneumoniae choosed tertiary hospitals for treatment,while 15(88.24%)children with Mycoplasma pneumoniae used macrolide antibiotics.CONCLUSION Children aged 4~7 years were susceptible to Mycoplasma pneumoniae infection among children aged at O~14.Children with Mycoplasma pneumoniae had a longer course of illness,and non-standard use of macrolide antibacterial drugs was widespread.
