核心肌群稳定性训练联合呼吸功能锻炼对胸腰椎压缩性骨折临床疗效的研究

Study on the Clinical Effect of Core Muscle Stability Training Combined with Respiratory Function Exercise on Thoracolumbar Compression Fractures

摘要 目的:探讨核心肌群稳定性训练联合呼吸功能锻炼对胸腰椎压缩性骨折患者的干预效果。方法:将60例胸腰椎压缩性骨折患者随机分组。对照组术后开展常规康复宣教与传统物理治疗,观察组术后进行核心肌群稳定性训练联合呼吸功能锻炼干预。比较两组术后2周与术后3个月疼痛程度、腰椎功能、生活自理能力、腰椎活动度、伤椎康复情况,比较两组术后3个月内并发症发生率。结果:相较对照组,观察组术后3个月视觉模拟评分法(VAS)评分、功能障碍指数问卷表(ODI)评分明显更低,Barthel指数(ADL)评分明显更高(P<0.05)。相较对照组,观察组术后3个月腰椎活动度明显更高(P<0.05)。相较对照组,观察组术后3个月Cobb角、伤椎后凸角明显减少,伤椎前缘高度明显升高(P<0.05)。并发症发生风险比较,观察组明显更低(P<0.05)。结论:核心肌群稳定性训练联合呼吸功能锻炼对胸腰椎压缩性骨折患者的干预效果较好,可降低患者术后疼痛程度,提升其腰椎功能,改善患者生活自理能力,加速伤椎康复进度,且患者术后并发症发生风险较低,值得临床推广。 Objective:To explore the intervention effect of core muscle stability training combined with respiratory function exercise on patients with thoracolumbar compression fractures.Methods:60 patients with thoracolumbar compression fracture were randomly grouped.The control group underwent routine rehabilitation education and traditional physical therapy,and the observation group underwent core muscle stability training combined with respiratory function exercise.Pain degree,lumbar function,self-care ability,lumbar motion and rehabilitation of injured vertebrae were compared between the two groups 2 weeks and 3 months after surgery,and the incidence of complications within 3 months after surgery was compared between the two groups.Results:Compared with the control group,the visual analogue scale(VAS)score and the Disability Index questionnaire(ODI)score were significantly lower in the observation group 3 months after surgery,and the Barthel index(ADL)score was significantly higher(P<0.05).Compared with the control group,the lumbar motion of the observation group was significantly higher at 3 months after surgery(P<0.05).Compared with the control group,the Cobb Angle and kyphotic Angle of the injured vertebra were significantly reduced in the observation group 3 months after surgery,and the height of the injured vertebra was significantly increased(P<0.05).The risk of complications was significantly lower in the observation group(P<0.05).Conclusion:Core muscle stability training combined with respiratory function exercise has a good effect on the intervention of patients with thoracolumbar compression fractures,which can reduce the degree of postoperative pain of patients,improve their lumbar function,improve their self-care ability,accelerate the rehabilitation progress of injured vertebrae,and the risk of postoperative complications of patients is low,which is worthy of clinical promotion.
