摘要
目的:探讨认知功能障碍训练联合通督醒脑头针治疗小儿脑性瘫痪(cerebral palsy,CP)合并智力障碍的临床疗效。方法:本研究为前瞻性研究,采用随机数字表法将宁夏回族自治区人民医院2020年2月至2025年2月期间收治的70例CP合并智力障碍的患儿分为对照组(n=35)和治疗组(n=35)。对照组接受认知功能障碍训练,治疗组接受认知功能障碍训练联合通督醒脑头针治疗。比较两组疗效、症状评分和血流动力学指标。结果:与对照组相比,治疗组的临床总有效率更高(P<0.05)。治疗后,治疗组格塞尔发育诊断量表(gesell development diagnosis scale,GDDS)、语言发育迟缓检查法(sign-significant language development delay examination,S-S)评分和粗大运动功能评定量表88项(gross motor function measure-88,GMFM-88)评分高于对照组(P<0.05)。治疗后,治疗组大脑前动脉血流速度、大脑中动脉血流速度和大脑后动脉血流速度高于对照组,血管搏动指数低于对照组(P<0.05)。结论:通督醒脑头针联合认知功能训练治疗CP合并智力障碍患儿疗效显著,可全面提升其智力水平、语言功能和运动能力,考虑与调节患儿的脑部血流动力学有关。
Objective:To explore the clinical efficacy of cognitive dysfunction training combined with Tongdu Xing Naotou scalp acupuncture in the treatment of children with cerebral palsy(CP)complicated with intellectual disability.Methods:This study was a prospective study,70 children with CP complicated with intellectual disability admitted to Ningxia Hui Autonomous Region People's Hospital from February 2020 to February 2025 were divided into the control group(n=35)and the treatment group(n=35)by random number table method.The control group received cognitive dysfunction training while treatment group received cognitive dysfunction training combined with Tongdu Xingnao scalp acupuncture.The efficacy,symptom score and hemodynamic indexes in two groups were compared.Results:Compared with control group,the total clinical effective rate of the treatment group was higher(P<0.05).After treatment,the scores of gesell developmental diagnostic scale(GDDS),sign-significant language development delay examination(S-S)and gross motor function measure scale-88(GMFM-88)in treatment group were higher than those in control group(P<0.05).After treatment,the blood flow velocity of anterior cerebral artery,the blood flow velocity of middle cerebral artery and the blood flow velocity of posterior cerebral artery in treatment group were higher than those in control group,and the vascular pulsation index was lower than that in control group(P<0.05).Conclusion:Tongdu Xingnao scalp acupuncture combined with cognitive function training is effective in the treatment of children with CP and intellectual disability,witch can comprehensively improve their intelligence level,language function and motor ability,and is considered to be related to the regulation of children's brain hemodynamics.
