摘要
目的探讨血清C反应蛋白(CRP)、D-二聚体水平联合肺部超声在新生儿呼吸窘迫综合征(NRDS)中的诊断价值。方法选取2022年9月至2024年3月福建省漳州正兴医院收治的86例NRDS患儿作为研究组,同期50例健康新生儿作为对照组。所有新生儿均进行血清CRP、D-二聚体水平检测,并接受肺部超声检查。比较两组CRP、D-二聚体水平及肺部超声表现的差异,并采用受试者工作特征(ROC)曲线分析其诊断效能。结果研究组血清CRP、D-二聚体水平高于对照组,差异有统计学意义(P<0.05)。肺部超声显示,研究组肺部异常率高于对照组,差异有统计学意义(P<0.05)。ROC曲线分析显示,血清CRP、D-二聚体联合肺部超声诊断新生儿呼吸窘迫综合征的AUC为0.993,均高于各单项指标的AUC。联合预测的敏感度和特异度分别为0.980、0.930。结论血清CRP、D-二聚体水平联合肺部超声检查诊断NRDS的效能良好,为临床早期识别、及时干预提供了有力支持。
Objective To explore the diagnostic value of serum C-reactive protein(CRP)and D-dimer levels combined with lung ultrasound in neonatal respiratory distress syndrome(NRDS).Methods A total of 86 children with NRDS admitted to Zhangzhou Zhengxing Hospital from September 2022 to March 2024 were selected as the study group,and 50 healthy newborns during the same period were selected as the control group.All newborns underwent tests for serum CRP and D-dimer levels and received lung ultrasound examinations.The differences in CRP,D-dimer levels and lung ultrasound manifestations between the two groups were compared,and the diagnostic efficacy was analyzed by using the receiver operating characteristic(ROC)curve.Results The levels of serum CRP and D-dimer in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Lung ultrasound showed that the abnormal rate of the lungs in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).ROC curve analysis showed that the AUC of serum CRP,D-dimer combined with lung ultrasound in the diagnosis of neonatal respiratory distress syndrome was 0.993,which was all higher than the AUC of each individual index.The sensitivity and specificity of the combined prediction were 0.980 and 0.930,respectively.Conclusion The combination of serum CRP and D-dimer levels and lung ultrasound examination has good efficacy in diagnosing NRDS,providing strong support for early clinical identification and timely intervention.
