摘要
目的基于Kano模型分析宫腔镜日间手术患者延续护理的需求。方法采取便利抽样法,选取2023年5月—2024年9月在湖州市某三甲医院治疗的400例宫腔镜日间手术患者为研究对象。采用基于Kano模型及德尔菲专家咨询法设计的问卷进行调查。使用Kano模型进行需求属性分类,通过满意影响力系数-不满意影响力(better coefficient-worse coefficient,Better-Worse)系数分析及二维矩阵分析确定需求属性,并通过需求筛选最敏感和需要改进的要素进行策略优化。结果宫腔镜日间手术患者延续护理的需求中,期望需求有10个条目(52.63%),必备需求有6个条目(31.58%),魅力需求有3个条目(15.79%)。根据支持需求进行敏感性分析,其中敏感度最高为0.442,最低为-0.185。其中条目15“随访方式”、条目3“活动指导”、条目1“疼痛缓解”是敏感度最高的3个需求。结论医护工作者应将随访方式个性化、活动指导精准化及疼痛管理标准化作为延续护理方案优化的重点,以更有效地满足患者核心需求,提升术后康复质量。
Objective To analyze the needs for continuous nursing care among patients undergoing hysteroscopic day surgery based on the Kano model.Methods A convenience sampling method was adopted to select 400 hysteroscopic day surgery patients treated in a tertiary hospital in Huzhou from May 2023 to September 2024 as the research subjects.A survey was conducted using a questionnaire designed based on the Kano model and the Delphi expert consultation method.The Kano model was used to classify demand attributes,which were determined through better coefficient-worse coefficient(Better-Worse)analysis and two-dimensional matrix analysis.The most sensitive elements and those requiring improvement were identified for strategic optimization.Results Among the needs for continuous nursing care among patients undergoing hysteroscopic day surgery,there were 10 items(52.63%)of expected needs,6 items(31.58%)of must-be needs,and 3 items(15.79%)of attractive needs.Sensitivity analysis based on the support needs revealed the highest sensitivity was 0.442 and the lowest was-0.185.Among them,item 15("follow-up method"),item 3("activity guidance"),and item 1("pain relief")were the three needs with the highest sensitivity.Conclusion Medical workers should focus on optimizing continued care plans with personalized follow-up methods,precise activity guidance,and standardized pain management to more effectively meet the core needs of patients and improve the quality of postoperative rehabilitation.
