摘要
目的探讨加味半夏白术天麻汤联合醒脑开窍针法对蛛网膜下腔出血脑血管痉挛患者的临床疗效。方法208例患者随机分为对照组、针刺组、中药组、联合组,每组52例,对照组给予常规治疗,针刺组在对照组基础上加用醒脑开窍针法,中药组在对照组基础上加用加味半夏白术天麻汤,联合组在对照组基础上加用加味半夏白术天麻汤联合醒脑开窍针法治疗,疗程21 d。检测临床疗效、颅内血流动力学指标(MCA-Vm、VICA-Vm、Lindegaard指数)、血管内皮功能指标(ET-1、VEGF、CGRP)、神经功能指标(mRS评分、MOCA评分)、中医证候评分、不良反应发生率变化。结果联合组总有效率高于对照组(P<0.05)。治疗后,4组颅内血流动力学指标、ET-1、VEGF、mRS评分、中医证候评分降低(P<0.05),CGRP、MOCA评分升高(P<0.05),以联合组更明显(P<0.05)。4组不良反应发生率比较,差异无统计学意义(P>0.05)。结论加味半夏白术天麻汤联合醒脑开窍针法可安全有效地调节蛛网膜下腔出血脑血管痉挛患者脑血流动力学,缓解临床症状,促进神经功能恢复。
AIM To explore the clinical effects of Supplemented Banxia Baizhu Tianma Decoction combined with Xingnao Kaiqiao acupuncture method on patients with cerebral vasospasm following subarachnoid hemorrhage.METHODS Two hundred and eight patients were randomly assigned into control group(52 cases)for 21-day intervention of conventional treatment,acupuncture group(52 cases)for 21-day intervention of both Xingnao Kaiqiao acupuncture method and conventional treatment,traditional Chinese medicine group(52 cases)for 21-day intervention of both Supplemented Banxia Baizhu Tianma Decoction and conventional treatment,and combination groups(52 cases)for 21-day intervention of Supplemented Banxia Baizhu Tianma Decoction,Xingnao Kaiqiao acupuncture method and conventional treatment.The changes in clinical effects,intracranial hemodynamic indices(MCA-Vm,VICA-Vm,Lindegaard index),vascular endothelial function indices(ET-1,VEGF,CGRP),neurological function indices(mRS score,MOCA score),TCM syndrome scores and incidence of adverse reactions were detected.RESULTS The combination group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the 4 groups displayed decreased intracranial hemodynamic indices,ET-1,VEGF,mRS score,TCM syndrome scores(P<0.05),and increased CGRP,MOCA score(P<0.05),especially for the combination group(P<0.05).No significant difference in incidence of adverse reactions was found among the 4 groups(P>0.05).CONCLUSION For the patients with cerebral vasospasm following subarachnoid hemorrhage,Supplemented Banxia Baizhu Tianma Decoction combined with Xingnao Kaiqiao acupuncture method can safely and effectively regulate cerebral hemodynamics,alleviate clinical symptoms,and promote neurological function recovery.
