摘要
目的 观察脓毒症相关性脑病(SAE)应用醒脑开窍针法治疗的临床效果。方法 选择80例SAE患者作为研究对象,均按随机数字表法分为观察组与对照组各40例。对照组给予西药常规对症治疗与液体复苏治疗、抗感染治疗、脏器功能支持治疗等,观察组在对照组基础上加用醒脑开窍针法治疗,两组均持续治疗7 d。对比两组临床疗效以及治疗前后的炎症指标[白细胞总数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)]以及神经损伤标志物[神经元特异性烯醇化酶(NSE)、S100钙结合蛋白β(S100β)]水平、急性生理学与慢性健康状况评价Ⅱ(APACHEⅡ)、序贯器官衰竭评分(SOFA评分)、格拉斯哥昏迷量表(GCS)评分。结果 观察组的总有效率较对照组显著升高(P<0.05)。治疗7 d后,观察组的WBC、IL-6、PCT、CRP、NSE、S100β因子水平及APACHEⅡ评分、SOFA评分相较对照组明显更低,且GCS评分更高(P<0.05)。结论 SAE患者应用醒脑开窍针法治疗可以改善神经功能并促进意识恢复,有助于减轻患者炎症并提升临床效果。
Objective:To observe the clinical efficacy of the Xingnao Kaiqiao acupuncture method in treating sepsis-associated encephalopathy(SAE).Methods:80 SAE patients admitted were selected and randomly assigned by a random number table method into an observation group and a control group,with 40 patients in each group.The control group received conventional western medical treatment,including symptomatic management,fluid resuscitation,anti-infective therapy,and organ function support.The observation group received the Xingnao Kaiqiao acupuncture method in addition to the same treatment as the control group.Both groups were treated for 7 days.Clinical efficacy,inflammatory markers[white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6)],neuronal damage markers[neuron-specific enolase(NSE),S100 calcium-binding proteinβ(S10oβ)],and clinical assessment scores[Acute Physiology and Chronic Health Evaluation II(APACHEⅡ),Sequential Organ Failure Assessment(SOFA),Glasgow Coma Scale(GCS)]were compared between the two groups before and after treatment.Results:The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).Compared with the control group,the observation group showed significantly lower levels of WBC,IL-6,PCT,CRP,NSE,and S100β,as well as lower APACHEⅡI and SOFA scores,and a higher GCS score after 7 days of treatment(P<O.05).Conclusion:The application of the Xingnao Kaiqiao acupuncture method in patients with SAE can improve neurological function and promote recovery of consciousness,help alleviate inflammation,and enhance clinical outcomes.
