摘要
目的:基于文献计量学方法分析机械通气(MV)病人呼吸机相关性肺炎(VAP)的研究现状及演进趋势。方法:检索Web of Science核心集与Scopus数据库自建库至2025年3月26日收录的VAP文献,运用VOSviewer 1.6.19和CiteSpace 6.3.R1软件进行国家合作网络、关键词共现/突发性探测及时区视图分析。结果:共纳入文献7 747篇,美国发文量居首(2 119篇),法国中介中心性最高(0.17),中国发文量排名第4位(521篇),但国际合作密度较低(中心性0.02);关键词共现分析显示,研究热点以“呼吸系统”(中心度0.35)和“感染控制”(中心度0.19)为核心;关键词突发性探测揭示,研究前沿聚焦“临床结局”“急性呼吸窘迫综合征(ARDS)”“体外膜肺氧合(ECMO)”“死亡率”和“序贯脏器衰竭评分(SOFA)”;时区视图显示VAP研究历经基础防控期(1986—2010年)、机制探索期(2009—2017年)和精准干预期(2019—2025年)三阶段演进趋势。结论:MV病人VAP研究呈现从单一防控向多学科整合的范式转变,未来需加强国际合作开发多组学数据的预测模型,开展不同医疗资源分层下的成本效益评估,构建“环境-宿主-病原体”三位一体防控体系。
Objective:To analyze the status quo and evolution trend of research on ventilator-associated pneumonia(VAP)in patients with mechanically ventilation(MV)using bibliometric methods.Methods:VAP literature was retrieved from the Web of Science Core Collection and Scopus database from its inception to March 2025.VOSviewer 1.6.19 and CiteSpace 6.3.R1 were used for national collaboration network,keyword co-occurrence/emergency detection,and time zone view analysis.Results:A total of 7,747 articles were included,with the United States having the most(2,119 articles),France having the highest centrality(0.17),and China ranking fourth in terms of number of articles(521 articles),but with a lower density of international collaborations(centrality 0.02).Keyword co-occurrence analysis showed that the research hotspots were"respiratory system"(centrality 0.35)and"infection control"(centrality 0.19).Keyword emergence detection revealed that the research frontier focused on"clinical outcomes,""acute respiratory distress syndrome(ARDS),""extracorporeal membrane oxygenation(ECMO),""mortality,"and"sequential organ failure score(SOFA)."The time zone view showed that VAP research has evolved through three stages:basic prevention and control(1986-2010),mechanism exploration(2009-2017),and precision intervention(2019-2025).Conclusion:VAP research in MV patients shows a paradigm shift from singlemode prevention to multidisciplinary integration.Future research should strengthen international collaboration to develop predictive models using multi-omics data,conduct cost-effectiveness assessments under different levels of medical resources,and construct a threedimensional prevention and control system integrating the environment,host,and pathogen.
