摘要
目的:观察分析彩色多普勒超声在痛风性关节炎早期诊断中的效果。方法:选取2020年1月~2023年12月来本院诊疗的50例痛风性关节炎患者设为观察组;另同期选择50例其他关节炎患者设为对照组,对比两组彩色多普勒超声检查结果;以及彩色多普勒超声诊断早期痛风性关节炎的灵敏度、特异度、阳性预测值、阴性预测值及诊断符合率。结果:观察组患者血流信号检出率高于对照组(P<0.05);观察组在检测痛风石、平行线以及关节腔内高回声点的表现上,其检出率高于对照组(P<0.05);两组在韧带内高回声以及骨侵蚀的检出率方面差异无统计学意义(P>0.05);彩色多普勒超声诊断早期痛风性关节炎的灵敏度为82.00%、特异度为66.00%、阳性预测值为72.81%、阴性预测值为77.91%、诊断符合率为74.00%。结论:彩色多普勒超声作为一种无创、高效的影像技术,在痛风性关节炎的早期诊断及与其他关节炎疾病的鉴别中具有重要的应用价值,可以提供丰富的血流和关节结构信息,诊断效能良好,有助于病情评估与早期干预。
Objective:To observe and analyze the effect of color Doppler ultrasound in the early diagnosis of gouty arthritis.Methods:50 patients with gouty arthritis who came to our hospital from January 2020 to December 2023 were selected as the observation group.At the same time,50 patients with other arthritis were selected as the control group,and the results of color Doppler ultrasound examination between the two groups were compared.And the sensitivity,specificity,positive predictive value,negative predictive value and diagnostic coincidence rate of color Doppler ultrasound in the diagnosis of early gouty arthritis.Results:The detection rate of blood flow signals in the observation group was higher than that in the control group(P<0.05).The observation group also demonstrated higher detection rates for tophi,parallel lines,and intra-articular high-echo points compared to the control group(P<0.05).However,there was no statistically significant difference between the two groups in the detection rates of ligamentous high-echo and bone erosion(P>0.05).The sensitivity of color Doppler ultrasound in diagnosing early gouty arthritis was 82.00%,with a specificity of 66.00%,positive predictive value of 72.81%,negative predictive value of 77.91%,and diagnostic concordance rate of 74.00%.Conclusion:Color Doppler ultrasound,as a noninvasive and efficient imaging technique,has important application value in the early diagnosis of gouty arthritis and the differentiation from other arthritis diseases,which can provide abundant information.
