摘要
目的分析颈动脉支架成形术(CAS)中使用闭环支架治疗不稳定斑块性颈动脉狭窄的疗效和相关并发症。方法回顾性分析2019年12月至2024年12月在本院接受颈动脉闭环支架成形术治疗不稳定斑块性颈动脉狭窄的患者的临床资料。结果共纳入31例不稳定斑块性颈动脉狭窄患者,所有患者共植入31枚颈动脉闭环支架,CAS术治疗前后的颈动脉平均狭窄程度分别为(64.3±9.7)%、(16.7±7.8)%,技术成功率为100%。在CAS术中,2例出现短暂性血管痉挛、4例出现心动过缓及低血压、1例出现轻度高灌注综合征,经相关治疗后患者的症状均好转。在CAS术后住院期间,1例患者出现小卒中(3.2%);在出院30天内,无患者出现卒中、心肌梗死及死亡;在出院30天后的随访期间,无患者发生同侧卒中或死亡,无患者出现支架变形、支架移位等其他并发症。结论闭环支架在治疗不稳定斑块性颈动脉狭窄中具有较高的技术成功率和较低的围手术期并发症发生率。
Objective To analyze the efficacy and related complications of using closed-cell stents in carotid artery stenting(CAS)for the treatment of carotid stenosis with unstable plaques.Methods Clinical data of patients with carotid stenosis due to unstable plaques who underwent CAS with closed-cell stents at our hospital from December 2019 to December 2024 were retrospectively analyzed.Results A total of 31 patients were enrolled,and 31 closed-cell carotid stents were implanted in all patients.The mean degree of carotid stenosis before and after CAS was(64.3±9.7)%and(16.7±7.8)%,respectively,with a technical success rate of 100%.During the CAS procedure,2 patients developed transient vasospasm,4 patients had bradycardia and hypotension,and 1 patient experienced mild hyperperfusion syndrome;all symptoms improved after corresponding treatment.During the postoperative hospital stay,1 patient(3.2%)suffered a minor stroke.Within 30 days after discharge,no patients had stroke,myocardial infarction,or death.During the follow-up period beyond 30 days after discharge,no patients occurred ipsilateral stroke or death,and no complications such as stent deformation or stent migration were observed.Conclusion The results of this study indicate that closed-cell stents have a high technical success rate and a low perioperative complication rate in the treatment of carotid stenosis with unstable plaques.However,as this is a preliminary exploratory study with a single center and small sample size,the above results need to be further verified in multi-center,large-sample prospective studies.
