摘要
野战外科是军事医学的基石,在具有大规模、长时间、对称性等特点的俄乌冲突中,战场伤员类型呈现复合化、群体化、重症化等特征,对野战外科能力提出了颠覆性挑战。源自伊拉克、阿富汗等非对称军事行动战场的“战术战伤救治(TCCC)-快速空运-损害控制手术”救治链不完全适用于未来战场,TCCC需转换为延时战伤救治,快速空运被迫转为陆空后送相结合,损害控制手术能力需与医疗队机动性相平衡。提升野战外科能力需要构建军地全面协同的战伤救治体系、生成战伤安全规范高效的救治能力和适应战场外科复苏的损害控制手术技能。
Field surgery serves as the cornerstone of military medicine,and field surgical capabilities centered on“damage control resuscitation and surgery”are important in reducing the mortality rate of combat casualties and maintaining combat effectiveness.In Russia-Ukraine conflict,characterized by large-scale,prolonged,and symmetrical engagements,battlefield injuries exhibit features of complexity,mass casualties,and severity,posing disruptive challenges to field surgical capabilities.The traditional chain of care——“Tactical Combat Casualty Care(TCCC)-rapid aeromedical evacuation-damage control surgery”——derived from military operations in Iraq and Afghanistan,is no longer fully applicable to future battlefields.TCCC must transition into prolonged casualty care,rapid aeromedical evacuation is compelled to shift toward a combined ground and air evacuation,and damage control surgery capabilities are constrained by mobility limitations.Enhancing field surgical capabilities requires establishing a comprehensive military-civilian collaborative system for combat casualty care,developing safe,standardized,and efficient combat casualty management capabilities,and adapting damage control surgery skills to meet the demands of future battlefield surgical resuscitation.
