摘要
甲状腺相关性眼病是与甲状腺疾病密切相关的眼部特异性自身免疫性疾病,临床表现主要有眼球突出、眼睑退缩、视力模糊、复视、结膜充血等,严重时可引发角膜溃疡,甚至会压迫视神经进而导致失明。目前,西医主要运用糖皮质激素、免疫抑制剂、激光及手术治疗,效果有限且不良反应严重。中医针药结合治疗有其独到优势,不仅能改善症状,对甲状腺功能亢进导致的高代谢也同样有效。本研究旨在基于玄府-目络瘀阻理论探索针药结合治疗甲状腺相关性眼病的有效方法,为临床治疗提供新思路与方法,提升疗效。文章全面梳理玄府-目络瘀阻理论在中医古籍中的起源、发展及其在眼科疾病治疗中的应用,同时系统总结现代医学对甲状腺功能亢进症(简称甲亢)突眼症发病机制、临床表现及治疗方法的认识,结合临床验案进行详细资料收集、记录治疗过程并评估治疗效果。通过将中医理论与临床实践相结合,深入剖析该病的发病机制,运用中药方剂调理全身气血,配合针灸治疗疏通玄府、目络,以达到改善症状、提高疗效的目的。研究发现,基于玄府-目络瘀阻理论,采用开通玄府、疏通目络的治疗原则,运用中药方剂与针灸的治疗方法,在改善甲状腺相关性眼病患者症状方面具有显著成效,验证了该理论在甲状腺相关性眼病治疗中的有效性与可行性。
Thyroid-associated ophthalmopathy(TAO),a specific autoimmune eye disease closely related to thyroid disorders,manifests clinically with symptoms including exophthalmos,eyelid retraction,blurred vision,diplopia,and conjunctival congestion.In severe cases,it can lead to corneal ulcers and even optic nerve compression resulting in blindness.Currently,Western medicine primarily employs glucocorticoids,immunosuppressants,laser therapy,and surgery,but these treatments have limited efficacy and significant adverse reactions.The traditional Chinese medicine(TCM)integrated therapy,combining acupuncture and Chinese medicine,demonstrates unique advantages,effectively ameliorating symptoms and managing the hypermetabolic state caused by hyperthyroidism.This study aims to explore effective treatment methods for TAO based on the theory of Xuanfu(玄府)-eye meridian obstruction,providing new perspectives and approaches for clinical treatment and enhancing TCM's therapeutic efficacy for this condition.This paper comprehensively reviews the origin and development of the Xuanfu-eye meridian obstruction theory in ancient Chinese medical literature and its application in ophthalmic diseases,and systematically summarizing modern medicine's understanding of the pathogenesis,clinical manifestations,and treatment methods for hyperthyroid exophthalmos.Detailed clinical case data collection,treatment process recording,and efficacy evaluation are conducted.By integrating TCM theory with clinical practice,the pathogenesis of this disease is deeply analyzed.Chinese medicine formulas are used to regulate systemic Qi and blood,combined with acupuncture therapy to clear Xuanfu and eye meridians,thereby ameliorating symptom and enhancing therapeutic effects.The study findings demonstrate that based on the theory of Xuanfu-eye meridian obstruction,the therapeutic principles of opening Xuanfu and clearing eye meridians,combined with Chinese medicine formulas and acupuncture treatments,show significant efficacy in ameliorating symptoms of TAO patients.This verifies the effectiveness and feasibility of this theory in treating thyroid-associated ophthalmopathy.
