寒热错杂型功能性消化不良患者脏腑红外热成像特征研究

摘要 目的 分析寒热错杂型功能性消化不良(FD)患者脏腑的红外热成像特征。方法 本研究共纳入106例观察对象,其中2018年6月—2020年6月就诊于北京市宣武中医医院肝胆脾胃病科的寒热错杂型FD患者作为观察组(53例),非FD的平和体质健康人群作为对照组(53例)。采用红外成像仪检测2组脏腑、任脉督脉对应体表位置的相对温度差,观察寒热错杂型FD患者脏腑、任脉督脉的相对温度差特点。结果 2组胃、小肠、左肾对应区观察组相对温度差高于对照组(P<0.05),左肺、右肺、左大肠对应区观察组相对温度差小于对照组(P<0.05);观察组中焦前相对温度差大于对照组、下焦后相对温度差小于对照组,差异有统计学意义(P<0.05);观察组任脉穴位中庭、鸠尾、巨阙、上脘、中脘、建里、下脘、水分、神阙后、阴交相对温度差大于对照组,差异有统计学意义(P<0.05);观察组督脉穴位中枢、脊中相对温度差大于对照组,差异有统计学意义(P<0.05)。结论寒热错杂型功能性消化不良患者的红外热成像主要表现为胃热脾寒肠寒,因虚致实,脾胃气滞,肺肾同病,病位在中焦,可波及下焦,其经络病机为任督二脉经气开阖不利。 Objective To analyze the infrared thermographic characteristics of the organs in patients with functional dyspepsia with cold and heat in complexity syndrome.Method A total of 106 participants were included in this study.From June 2018 to June 2020,53 patients with functional dyspepsia of cold and heat in complexity syndrome,who visited the Department of Spleen and Stomach Diseases at Beijing Xuanwu Traditional Chinese Medicine Hospital,were enrolled as the observation group,and 53 healthy individuals with balanced constitution were recruited as the control group.An infrared thermography imager was used to detect the relative temperature differences at body surface locations corresponding to the viscera and the conception and governor vessels in both groups.The relative temperature differences of the viscera and conception and governor meridian points were compared between the observation and control groups.Result The relative temperature differences in the stomach,small intestine,and left kidney regions were higher in the observation group than in the control group(P<0.05),whereas the left lung,right lung,and left large intestine regions were lower(P<0.05).In the observation group,the anterior middle energizer region showed higher relative temperature differences than the control group,while the posterior lower energizer region was lower,with statistically significant differences(P<0.05).For conception vessel points Zhongting(CV16),Jiuwei(CV15),Juque(CV14),Shangwan(CV13),Zhongwan(CV12),Jianli(TE14),Xiawan(CV10),Shuifen(CV9),posterior Shenque(CV8),and Yinjiao(CV7),the observation group exhibited higher relative temperature differences than the control group(P<0.05).For governor meridian points Zhongshu(GV7)and Jizhong(GV6),the observation group showed higher relative temperature differences compared to the control group(P<0.05).Conclusion Infrared thermography of patients with functional dyspepsia of cold and heat in complexity syndrome mainly presents as stomach heat,spleen cold,and intestinal cold,caused by deficiency leading to consolidation,spleen and stomach qi stagnation,and concurrent lung and kidney involvement.The pathological changes are primarily located in the middle energizer and may affect the lower energizer,and the meridian and collateral mechanism involves unfavorable opening and closing of the conception and governor vessels.
