摘要
目的 探讨感知配偶支持在妇科癌症患者双向沟通质量与家庭弹性间的中介作用。方法采用便利抽样法,选择2021年5月至2023年12月妇科癌症患者148例为研究对象,采用亲密关系支持问卷(SIRRS)评价感知配偶支持水平;家庭弹性评定量表中文简化版(FRAS-C)完成患者家庭弹性评估;采用妇科癌症患者夫妻沟通质量量表评估双向沟通质量;选择Pearson相关性软件完成不同变量间系数计算,构建结构方程,完成数据拟合后并判断是否存在中介效应。结果 妇科癌症患者SIRRS量表得分以妻子感受到配偶支持得分最高(31.24±4.42)分;FRAS-C量表中家庭沟通与解决问题得分为(72.59±6.31)分;双向沟通质量得分以压力应对和创造常态得分最高;相关性结果表明,SIRRS量表得分与FRAS-C量表得分及双向沟通质量量表各维度得分均呈负相关性(P<0.05);家庭弹性中家庭沟通与问题解决、利用社会资源和持有积极看法与双向沟通质量各维度得分呈负相关性(P<0.05);结构方程模型结果表明,双向沟通质量对家庭弹性时95%置信区间为(0.045,2.041),双向沟通质量主要通过感知配偶支持的亲密关系的中介作用影响家庭弹性。结论 妇科癌症患者双向沟通水平偏低,感知配偶支持一般,家庭弹性较差,且结构方程模型结果说明双向沟通质量可通过感知配偶支持对家庭弹性发挥间接预测作用。
Objective To explore the mediating effect of perceived spouse support on the quality of two-way communication and family flexibility in gynecological cancer patients.Methods One hundred and forty-eight gynecological cancer patients from May 2021 to December 2023 were selected by convenience sampling method,and the perceived spouse support level was evaluated by Support in Intimate Relationships Rating Scale(SIRRS).The simplified Chinese version of Family Flexibility Rating Scale(FRAS-C)was used to evaluate the family flexibility of patients.The quality of two-way communication was evaluated by the marital communication quality scale of gynecological cancer patients.Pearson correlation software was selected to calculate the coefficient between different variables,construct the structural equation,and determine whether the intermediary effect existed after data fitting.Results Women with gynecological cancer had the highest SIRRS score(31.24±4.42)points.The score of family communication and problem solving in FRAS-C scale was(72.59±6.31)points;the two-way communication quality score was the highest in stress coping and normalcy creation.The correlation results showed that SIRRS scale was negatively correlated with FRAS-C scale and bidirectional communication quality scale(P<0.05).In family flexibility,family communication and problem solving,using social resources and holding positive views were negatively correlated with the quality of two-way communication(P<0.05).The structural equation model results show that the 95%confidence interval of two-way communication quality on family flexibility is(0.045,2.041),and two-way communication quality affects family elasticity mainly through the mediating effect of perceived spousal-supported intimate relationship.Conclusions Patients with gynecological cancer have low level of two-way communication,average perceived spousal support,and poor family flexibility,and structural equation model results indicate that two-way communication quality can indirectly predict family flexibility through perceived spousal support.
