摘要
目的:评估基于心排血指数(CI)的个性化触发延迟技术对下肢动脉CT血管造影(CTA)的成像质量的影响。方法:前瞻性纳入2022年7月至2025年3月哈尔滨二四二医院收治的143例下肢缺血患者,采用随机数表法将其分为观察组(72例)和对照组(71例),观察组CTA扫描采用个性化触发延迟,对照组CTA扫描采用固定触发延迟(25 s)。采用无创心输出量监测设备(USCOM®)测量CI,并构建CI对比剂达峰时间(T)模型,动态调整CTA扫描时相。对比分析两组图像优良率、静脉污染率、CT值、信噪比(SNR)及心功能分级中Ⅲ~Ⅳ级患者的图像优良率差异。结果:观察组动脉显影优良率为90.3%,显著高于对照组的64.8%,静脉污染率为8.3%,低于对照组的29.6%,两组比较差异均有统计学意义(x^(2)=15.2、11.8,P<0.01)。观察组股动脉CT值和SNR均值分别为(320±45)HU和25.3±4.1,显著高于对照组,差异均有统计学意义(t=-6.387、7.409,P<0.01)。观察组心功能分级中Ⅲ~Ⅳ级患者图像优良率为92.3%,优于对照组,差异有统计学意义(U=49.235,P<0.01)。结论:个性化触发延迟技术通过精准匹配个体化血流动力学特征,可显著优化下肢CTA成像质量,为心功能异常患者提供了安全高效的下肢动脉CTA检查方案。
Objective:To assess the impact of personalized trigger delay technique based on cardiac index(CI)on imaging quality of computed tomography angiography(CTA)for lower limb artery.Methods:A total of 143 patients with lower limb ischemia who admitted to Harbin 242 Hospital from July 2022 to March 2025 were prospectively included.They were randomly divided into an observation group(72 cases)and a control group(71 cases)using a random number table method.The CTA scan of observation group adopted personalized trigger delay,and that of control group adopted the fixed trigger delay(25 s).The non-invasive cardiac output monitoring equipment(USCOM®)was adopted to measure CI,and to construct CI-contrast agent peak time(T)model to dynamically adjust CTA scanning phase.The differences of the excellent rate of image,contamination rate of vein,signal-to-noise ratio(SNR),and the excellent rate of image of patients at grade III-IV of cardiac function grading between two groups were compared and analyzed.Results:The excellent rate of arterial imaging was 90.3%in the observation group,which was significantly higher than that(64.8%)of the control group.The contamination rate of vein was 8.3%in the observation group,which was significantly lower than that(29.6%)of the control group.The differences of the above two indicators between two groups were statistically significant(x^(2)=15.2,11.8,P<0.01).The CT value and signal-to-noise ratio(SNR)of the femoral artery were respectively(320±45)HU and 25.3±4.1 in the observation group,which were significantly higher than those in the control group,and the differences of them between two groups were significant(t=-6.387,7.409,P<0.01).The excellent rate of images of patients at grade III-IV of cardiac function grading in the observation group was 92.3%,which was better than that(53.8%)in the control group,and the difference was statistically significant(U=49.235,P<0.01).Conclusion:Personalized trigger delay technique can significantly optimize the quality of CTA imaging for lower limb by accurately matching personalized hemodynamic characteristics,which can provide safe and efficient CTA examination plan for lower limb artery of patients with cardiac dysfunction.
