摘要
本文基于“土郁夺之”理论,对肥胖症病机特点及治疗思路进行探讨。肥胖症是一种体内脂肪过度蓄积的慢性代谢性疾病,主要病位在脾胃,基本病机为“土郁”。土实则能食,膏脂堆积,土虚则运化无力,痰湿内停,阻滞气机,最终膏脂、痰湿壅滞引起体重增加,导致肥胖,膏脂、痰湿、气血壅滞,进一步损伤脏腑组织络脉,导致肥胖进展及其变证。针对“土郁”采用“夺之”之法,包括清胃热、补脾气、消食导滞、祛湿化痰、化浊降脂、行气疏肝、温阳利水、逐瘀降浊等治法,祛除病理产物,恢复脾胃功能,运化堆积之膏脂、痰湿,从而减轻体重,使“土郁夺之”理论为指导肥胖症辨证施治提供参考。
Based on the theory of"earth depression and depletion",this paper explores the pathogenic characteristics and therapeutic approaches of obesity.Obesity is a chronic metabolic disease characterized by excessive accumulation of body fat,with the spleen and stomach as the primary sites of pathology and"earth depression"as the fundamental pathogenesis.When the earth element is excessive,food intake is abundant and fats accumulate;when it is deficient,transformation and transportation are impaired,leading to the internal retention of phlegm and dampness,obstruction of qi movement,and ultimately weight gain and obesity.The stagnation of fat,phlegm,dampness,and qi and blood further damages the viscera,tissues,meridians,and collaterals,resulting in the progression of obesity and its associated disorders.In response to"earch depression",the therapeutic principle of"depletion"is applied,including clearing stomach heat,tonifying spleen qi,eliminating food stagnation,dispelling dampness and resolving phlegm,transforming turbidity and reducing lipids,regulating qi and soothing the liver,warming yang and promoting diuresis,and dispelling blood stasis and reducing turbidity.These approaches eliminate pathological products,restore spleen and stomach function,and promote the transformation and transportation of accumulated fat and phlegm-dampness,thereby reducing body weight.The theory of"earth depression and depletion"thus provides a reference for guiding syndrome differentiation and treatment of obesity.
