摘要
压力性尿失禁(SUI)是指患者在做咳嗽、大笑以及打喷嚏等腹压增高的运动时,出现尿液不自主流出的一种疾病,好发于产后及中老年女性,且常伴焦虑、抑郁等异常情绪,极大影响患者的身心健康。本团队基于形气神三位一体生命观探讨SUI合并焦虑、抑郁状态的发病机制及治疗,从形、气、神三个维度依次展开辨析,提出“形损为发病之基、气乱为发病之枢、神伤为发病之本”的病机,以理形、调气、治神为治疗原则,予以手法正骨、调曲,快速改善患者症状;内服中药疏肝健脾补肾、调畅气机;针刺加艾灸平衡阴阳、调控情志。由此可多维度、全面、系统地治疗女性SUI合并焦虑、抑郁状态,以期为临床治疗提供新方向和思路。
Stress urinary incontinence(SUI)refers to a condition in which patients experience involuntary leakage of urine during activities that increase intra-abdominal pressure,such as coughing,laughing,and sneezing.It is commonly seen in postpartum and middle-aged to elderly women and is often accompanied by abnormal emotions such as anxiety and depression,significantly impacting patients'physical and mental well-being.Our team has explored the pathogenesis and treatment of female SUI complicated with anxiety and depression based on the body-qi-spirit trinity life concept.We have conducted analyses successively from the three dimensions of body,qi,and spirit,proposing the pathogenesis that"damage to the body serves as the foundation of disease onset,qi disharmony acts as the pivot of disease onset,and spirit impairment is the root cause of disease onset."With the treatment principles of regulating the body,harmonizing qi,and treating the spirit,we employ manual bone-setting and curve adjustment techniques to rapidly alleviate patients'symptoms;oral administration of Chinese medicines to soothe the liver,strengthen the spleen,tonify the kidneys,and regulate qi movement;and acupuncture combined with moxibustion to balance yin and yang and regulate emotions.This approach enables multidimensional,comprehensive,and systematic treatment of female SUI complicated with anxiety and depression,aiming to provide new directions and insights for clinical treatment.
