摘要
目的:探讨手法正骨硬纸夹板合石膏外固定治疗伸直型儿童肱骨髁上骨折的疗效。方法:回顾性分析2020年1月至2024年1月行正骨治疗的伸直型儿童肱骨髁上骨折患者52例,根据采用的固定方法分为硬纸夹板石膏组和单纯石膏组,比较两组患者治疗后影像学指标、骨折愈合时间、并发症发生率差异,参照Flynn评价标准评价疗效。结果:治疗后3个月根据Flynn评价标准硬纸夹板石膏组优良率为92.59%,单纯石膏组为88.00%,差异有统计学意义(P<0.05);单纯石膏组患者治疗后1周及治疗后3个月Baumann角与治疗后即刻比较差异均有统计学意义(P<0.05),硬纸夹板石膏组差异无统计学意义(P>0.05);各组治疗后3个月与治疗后即刻肱骨前线通过肱骨头中1/3情况比较差异均有统计学意义(P<0.05)。结论:儿童肱骨髁上骨折在保证手法复位质量的基础上,用硬纸夹板合石膏外固定治疗可获得良好疗效,且疗效优于单纯石膏外固定。
Objective:To explore the therapeutic effect of manual reduction with hard paper splint combined with gypsum external fixation in the treatment of straight ype humeral supracondylar fractures in children.Methods:A retrospective analysis was conducted on 52 children with straight type supracondylar fractures of the humerus who underwent orthopedic treatment from January 2020 to January 2024.The patients were divided into two groups based on the different fixation methods used:the hard paper splint and gypsum group and the gypsum group.The differences in imaging indicators,fracture healing time,and incidence of complications between the two groups after treatment were compared.and the efficacy was evaluated according to the Flynn elbow joint function evaluation criteria.Results:3 months after treatment,according io the Flynn evaluation criteria.the excellent and good rate of the hard paper splint and gypsum group was 92.59%.while the gypsum group was 88.00%,with a statistically significant difference(P<0.05),There were statistically significant differences(P<0.05)in Baumann angles between the gypsum group at 1 week and 3 months after treatment and immediately after treatment,while there was no difference(P>0.05)in the hard paper splint and gypsum group.There were sig nificant differences(P<0.05)in the passage of the humeral front linc through 1/3 of the humeral head between each group after 3 months of treatment and immediately after treatment.Conclusion:On the basis of ensuring the quality of manual reduction.the use of hard paper splints combined with gypsum external fixation can achieve good therapeutic effects for children with supracondylar fractures of the humerus,and the therapeutic effect is better than that of simple gypsum external fixation.
