摘要
目的 探讨不同级别软骨肉瘤的CT征象。方法 选取行CT检查的50例软骨肉瘤患者,记录软骨肉瘤的CT征象,比较不同级别、病理类型软骨肉瘤的CT征象。结果 50例软骨肉瘤患者中,CT检查检出软组织肿块43例(86.00%),骨质破坏45例(90.00%),钙化41例(82.00%),细微钙化21例(42.00%),骨髓水肿41例(82.00%),骨皮质增厚24例(48.00%),骨膨胀42例(84.00%),骨膜反应26例(52.00%)。50例软骨肉瘤患者中,低级别20例,中高级别30例。CT检查显示,中高级别软骨肉瘤软组织肿块和骨髓水肿检出率均高于低级别软骨肉瘤,细微钙化检出率低于低级别软骨肉瘤,差异均有统计学意义(P﹤0.05)。CT检查共检出48例软骨肉瘤,检出率为96%,具体类型为普通髓腔型29例,黏液型9例,间质型6例,透明细胞型4例。结论 CT检查在软骨肉瘤的诊断和术前分级中具有重要的临床价值,尤其是在评估肿瘤的侵袭性和生物学行为方面。通过准确分析CT征象,临床医师可以更好地了解软骨肉瘤的类型和分级,从而制订个体化治疗方案,改善患者预后。
Objective To analyze the CT signs in chondrosarcoma of different grades.Method Fifty patients with chondrosarcoma who underwent CT examination were selected.The CT signs of chondrosarcoma were recorded,and the CT signs in chondrosarcoma of different grades and pathological types were compared.Result Among the 50 patients with chondrosarcoma,CT examination detected 43 cases(86.00%)of soft tissue masses,45 cases(90.00%)of bone destruction,41 cases(82.00%)of calcification,21 cases(42.00%)of fine calcification,41 cases(82.00%)of bone marrow edema,24 cases(48.00%)of thickened cortical bone,42 cases(84.00%)of bone expansion,and 26 cases(52.00%)of periosteal reaction.Among the 50 patients with chondrosarcoma,20 cases were low grade and 30 cases were medium to high grade.CT examination showed that the detection rates of soft tissue masses and bone marrow edema in medium and high-grade chondrosarcoma were both higher than those in low-grade chondrosarcoma,while the detection rate of fine calcification was lower than that in low-grade chondrosarcoma,the differences were statistically significant(P<0.05).A total of 48 cases of chondrosarcoma were detected by CT examination,with a detection rate of 96%.The specific types included 29 cases of common medullary cavity type,9 cases of mucinous type,6 cases of interstitial type,and 4 cases of clear cell type.Conclusion CT examination has significant clinical value in the diagnosis and preoperative grading of chondrosarcoma,especially in evaluating the invasiveness and biological behavior of tumors.By accurately analyzing CT signs,clinicians can better understand the types and clinical grades of chondrosarcoma,thereby formulating individualized treatment plans and improving the prognosis of patients.
