摘要
多发性肌炎/皮肌炎合并间质性肺疾病病因尚未明确,结合中医经典相关论述,认为其临床表现与中医五脏中的肺脾相联系。本文从脏腑功能、经络、五行方面阐述肺脾间“同气相求”关系。基于“同气相求”理论,从外邪侵袭、肺脾损伤两方面探讨该病的病因病机,并分阶段(急性发作期、缓解期)提出相应治法,为中医药治疗多发性肌炎/皮肌炎合并间质性肺疾病提供理论支撑,为临床治疗该疾病提供新的思路。
The etiology of polymyositis/dermatomyositis combined with interstitial lung disease has not been clarified.Combined with the relevant traditional Chinese medicine(TCM)classics,it is believed that its clinical manifestations can be related to the lung and spleen in the five viscera of TCM.The theory of“origin-analogue”plays an important role in TCM.This paper expounds the“origin-analogue”relationship between lung and spleen from the aspects of zang-fu function,meridians and five elements.Based on the theory of“origin-analogue”,the etiology and pathogenesis of the disease are explored from the two aspects of exogenous pathogen invasion and lung and spleen injury,and corresponding treatment methods are proposed in different stages(acute attack stage and remission stage),providing theoretical support for the TCM treatment of polymyositis/dermatomyositis combined with interstitial lung disease,and providing new ideas for the clinical treatment of the disease.
