摘要
目的:分析经皮空心钉联合改良滑动加压动力髋螺钉内固定术治疗股骨颈骨折的效果。方法:选取2022年1月—2024年6月曹县人民医院骨外科收治的股骨颈骨折患者88例作为研究对象,根据治疗方案不同分为对照组(实施经皮空心钉内固定术)与观察组(实施经皮空心钉联合改良滑动加压动力髋螺钉内固定术),各44例。比较两组手术效果。结果:两组手术时间、术中出血量比较,无统计学差异(P>0.05);观察组住院时间短于对照组,骨折愈合时间早于对照组(P<0.001)。术后1个月、3个月,两组髋伸屈活动度、髋内旋外旋活动度提高,且观察组高于对照组(P<0.001)。术后1个月、3个月,两组髋关节评分表、Barthel指数评分提高,且观察组高于对照组(P<0.001)。观察组并发症总发生率低于对照组(P=0.013)。结论:经皮空心钉联合改良滑动加压动力髋螺钉内固定术治疗股骨颈骨折的效果显著,有助于加速骨折端愈合,改善髋关节功能与日常生活活动能力,且手术并发症少,治疗安全性高。
Objective:To analyze the therapeutic effect of percutaneous hollow nail combined with modified sliding compression dynamic hip screw internal fixation in the treatment of femoral neck fractures.Methods:A total of eighty-eight patients with femoral neck fractures who were admitted to the department of orthopedic surgery of Caoxian County People's Hospital from January 2022 to June 2024 were selected as the research subjects.They were divided into the control group(treated with percutaneous hollow nail internal fixation)and the observation group(treated with percutaneous hollow nail combined with modified sliding compression dynamic hip screw internal fixation)according to different treatment plans,with forty-four cases in each group.The surgical outcomes of the two groups were compared.Results:There was no statistically significant difference in the operation time and intraoperative blood loss between the two groups(P>0.05).The hospitalization time of the observation group was shorter than that of the control group,and the fracture healing time was earlier than that of the control group(P<0.001).One month and three months after the operation,the hip extension and flexion range of motion,as well as the hip internal and external rotation range of motion,increased in both groups,and the values in the observation group were higher than those in the control group(P<0.001).One month and three months after the operation,the scores of the hip joint assessment form and the Barthel index in both groups increased,and the scores of the observation group were higher than those of the control group(P<0.001).The total incidence of complications in the observation group was lower than that in the control group(P=0.013).Conclusion:The combined use of percutaneous hollow nail and modified sliding compression dynamic hip screws for internal fixation in the treatment of femoral neck fractures has a remarkable effect,which helps to accelerate the healing of the fracture site,improves hip joint function and daily living ability,and has fewer surgical complications and high treatment safety.
