摘要
目的分析脑血管实时3D模型路径图用于颅内动脉瘤介入治疗中的价值。方法选取2021年1月至2022年4月赣州市立医院收治的80例颅内动脉瘤患者作为研究对象,根据治疗方案的不同分为实验组与对照组,每组40例。对照组采用常规路径图引导下血管介入治疗,实验组采用实时3D模型路径图指导手术治疗。比较两组微导管一次塑形到位率、手术时间、机器射线量、造影剂使用量、术中破裂出血率、动脉瘤术后复发率及格拉斯哥昏迷量表(Glasgow coma scale,GCS)预后评分。结果实验组微导管一次塑形到位率为97.50%,高于对照组的80.00%,差异有统计学意义(P<0.05)。实验组手术时间短于对照组,一次手术机器射线总量、造影剂剂量均少于对照组,差异有统计学意义(P<0.05)。实验组完全栓塞率高于对照组,术中破裂出血率、术后动脉瘤复发率均低于对照组,差异有统计学意义(P<0.05)。实验组GCS预后评分高于对照组,差异有统计学意义(P<0.05)。结论实时3D路径图可提高动脉瘤手术质量,缩短手术时间,降低破裂出血量率和复发率,改善患者预后,值得临床推广应用。
Objective To analyze the value of real-time 3D model road map of cerebral vessels in interventional therapy of intracranial aneurysms.Methods A total of 80 patients with intracranial aneurysms admitted to Ganzhou Municipal Hospital from January 2021 to April 2022 were selected as research subjects,and they were divided into the experimental group and the control group according to different treatment plans,with 40 cases in each group.The control group received endovascular treatment under the guidance of a conventional roadmap,while the experimental group received surgical treatment under the guidance of a real-time 3D model road map.The terms of the one-time shaping success rate of microcatheters,operation time,machine radiation dose,contrast agent dosage,intraoperative rupture and bleeding rate,postoperative aneurysm recurrence rate and Glasgow coma scale(GCS)score were compared between the two groups.Results The one-time shaping success rate of microcatheters in the experimental group was 97.50%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).The operation time in the experimental group was shorter than that in the control group,and the total machine radiation dose and contrast agent dosage for a single operation were less than those in the control group,and the differences were statistically significant(P<0.05).The complete embolization rate in the experimental group was higher than that in the control group,while the intraoperative rupture and bleeding rate and postoperative aneurysm recurrence rate were lower than those in the control group,and the differences were statistically significant(P<0.05).The GCS score in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The real-time 3D model road map can improve the quality of aneurysm surgery,shorten the operation time,reduce the rupture bleeding rate and recurrence rate,and improve the prognosis of patients,it is worthy of clinical promotion and application.
