摘要
目的比较经脐单孔腹腔镜手术(TU-LESS)与传统多孔腹腔镜手术(MPLS)治疗卵巢良性肿瘤的效果。方法回顾性抽取2023年1月至2024年6月南阳市中心医院收治的91例行腹腔镜手术治疗的卵巢良性肿瘤患者,按不同手术入路分为TU-LESS组(47例)与MPLS组(44例)。比较两组手术相关指标、术后1 d疼痛情况(Prince-Herry评分法评分)、体象量表评分(BIS)、美容评分(CS)、术后并发症发生情况。结果两组失血量、排气时间、住院时间、术后白细胞、术后中性粒细胞、止疼药使用、切除附件、术中破裂情况比较差异未见统计学意义(P>0.05),但TU-LESS组手术时间长于MPLS组(P<0.05)。术后1 d,两组疼痛程度比较差异未见统计学意义(P>0.05);术后1、6个月,TU-LESS组BIS评分、CS评分均优于MPLS组(P均<0.05)。两组术后并发症发生率比较差异未见统计学意义(P>0.05)。结论TU-LESS腹腔镜手术治疗卵巢良性肿瘤效果良好,术后美容效果较好。
Objective To compare the efficacy of transumbilical single-port laparoscopic surgery(TU-LESS)and traditional multi-port laparoscopic surgery(MPLS)in the treatment of benign ovarian tumors.Methods Ninety-one patients with benign ovarian tumors who underwent laparoscopic surgery in Nanyang Central Hospital from January 2023 to June 2024 were retrospectively selected,and they were divided into the TU-LESS group(47 cases treated by TU-LESS)and the MPLS group(44 cases treated by MPLS)according to the surgery approach.The surgery-related indexes,1-day postoperative pain status(Prince-Herry score),body image scale score(BIS),cosmetic score(CS),and postoperative complication were compared between the two groups.Results There were no significant differences in blood loss,exhaust time,hospital stay,postoperative white blood cells,postoperative neutrophils,use of pain killers,excision of uterine appendage,and intraoperative rupture between the two groups(P>0.05).However,the operation duration in the TU-LESS group was longer than that in the MPLS group(P<0.05).There was no significant difference in pain degree between the two groups 1 day after surgery(P>0.05).The BIS score and CS score in the TU-LESS group were superior to those in the MPLS group 1 and 6 months after surgery(all P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions TU-LESS is effective in treating benign ovarian tumors,and it can effectively promote the aesthetic degree of the incision.
