摘要
目的观察鼻内镜手术联合化痰祛瘀开窍汤加减对慢性鼻-鼻窦炎患者自觉症状视觉模拟评分法(visual analogue scale,VAS)评分及鼻黏膜恢复情况的影响。方法选取2020年7月至2022年11月吉安市中心人民医院接诊的80例慢性鼻-鼻窦炎患者作为研究对象,根据随机数字表法分为对照组与观察组,每组40例。对照组行常规鼻内镜手术治疗,观察组加用化痰祛瘀开窍汤加减治疗。比较两组VAS评分、鼻内镜Lund-Kennedy评分、并发症发生情况及治疗效果。结果治疗后,观察组头痛、流涕、鼻塞与嗅觉障碍的VAS评分均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组息肉、水肿、鼻漏评分均低于对照组,差异有统计学意义(P<0.05);观察组术后血痂、黏脓涕、黏膜水肿肥厚发生率均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论鼻内镜手术联合化痰祛瘀开窍汤加减治疗慢性鼻-鼻窦炎,可有效改善头痛、流涕等症状,减轻疼痛不适感受,有利于促进鼻黏膜恢复效果,提升治疗效果。
Objective To observe the influence of endoscopic sinus surgery combined with modified Huatan Quyu Kaiqiao decoction on the visual analogue scale(VAS)score of subjective symptoms and the recovery of nasal mucosa in patients with chronic rhinosinusitis.Methods 80 patients with chronic rhinosinusitis who were admitted to Ji'an Central People's Hospital from July 2020 to November 2022 were selected as the research subjects,and they were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The control group was treated with routine endoscopic sinus surgery,while the observation group was treated with modified Huatan Quyu Kaiqiao decoction.The VAS scores,Lund-Kennedy scores of nasal endoscopy,occurrence of complications and therapeutic effects were compared between the two groups.Results After treatment,the VAS scores of headache,runny nose,nasal congestion and olfactory disorder in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);after treatment,the scores of polyps,edema and rhinorrhea in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);the incidences of postoperative scab,mucopurulent nasal discharge and mucosal edema and hypertrophy in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);the total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Endoscopic sinus surgery combined with modified Huatan Quyu Kaiqiao decoction for the treatment of chronic rhinosinusitis can effectively alleviate symptoms such as headache and runny nose,reduce pain and discomfort,and is conducive to promoting the recovery of nasal mucosa and enhancing the therapeutic effect.
