他克莫司联合抗真菌药物对假丝酵母菌体外抑制作用的研究

摘要 目的探讨他克莫司(TAC)分别与氟康唑(FLC)、伏立康唑(VRC)、两性霉素B(AMB)、卡泊芬净(CAS)联合对假丝酵母菌的体外抑制作用。方法白假丝酵母菌、热带假丝酵母菌、光滑假丝酵母菌、近平滑假丝酵母菌、克柔假丝酵母菌收集自青岛大学附属医院及康复大学青岛中心医院检验科。采用微量棋盘稀释法,测定FLC、VRC、AMB、CAS、TAC对白假丝酵母菌、光滑假丝酵母菌、热带假丝酵母菌、近平滑假丝酵母菌、克柔假丝酵母菌的最低抑菌浓度(MIC),并评估TAC分别联合4种抗真菌药物对上述5种假丝酵母菌的体外抑菌作用。结果单用TAC未观察到抗真菌作用,而TAC联合FLC对30.0%假丝酵母菌有协同作用,对24.0%假丝酵母菌有拮抗作用;TAC联合VRC对14.0%假丝酵母菌有协同作用,对10.0%假丝酵母菌有拮抗作用;TAC联合AMB对98.0%假丝酵母菌有协同作用,未观察到拮抗作用;TAC联合CAS对78.0%假丝酵母菌有协同作用,未观察到拮抗作用。结论TAC联合AMB、CAS在体外对假丝酵母菌有协同抑制作用,TAC联合FLC或VRC在体外对假丝酵母菌的协同率低并有拮抗作用。TAC联合AMB或CAS可能成为未来临床治疗侵袭性真菌病的新方案。 Objective To investigate the in vitro inhibitory effect of tacrolimus(TAC)combined with fluconazole(FLC),voriconazole(VRC),amphotericin B(AMB)or caspofungin(CAS)on Candida species.Methods Candida albicans,Candida tropicalis,Candida glabrata,Candida paraprata and Candida kerubicans were collected from the Laboratory Department of the Affiliated Hospital of Qingdao University and the Qingdao Central Hospital of Rehabilitation University.The microbroth checkerboard dilution method was used to determine the minimum inhibitory concentration(MIC)of FLC,VRC,AMB,CAS,and TAC against Candida albicans,Candida glabrata,Candida tropicalis,Candida parapsilosis,and Candida krusei,and the in vitro antibacterial effect of TAC combined with each of the above four antifungal agents on the above five Candida species were evaluated.Results No antifungal effect was observed for TAC used alone;however,TAC combined with FLC had a synergistic effect on 30.0%of Candida species and an antagonistic effect on 24.0%of Candida species;TAC combined with VRC had a synergistic effect on 14.0%of Candida species and an antagonistic effect on 10.0%of Candida species;TAC combined with AMB had a synergistic effect on 98.0%of Candida species and had no antagonistic effect;TAC combined with CAS had a synergistic effect on 78.0%of Candida species and had no antagonistic effect.Conclusion TAC combined with AMB or CAS has a sy-nergistic antibacterial effect against Candida species in vitro,and TAC combined with FLC or VRC has a low synergistic rate against Candida species in vitro and shows an antagonistic effect.TAC combined with AMB or CAS may become a new regimen for the future clinical treatment of invasive fungal disease.
