摘要
目的分析飞秒激光小切口角膜基质透镜取出术(SMILE)与飞秒激光辅助准分子激光原位角膜磨镶术(FS-LASIK)对近视患者角膜层间光密度、角膜前表面Q值的影响。方法回顾性病例研究。选取2023年7~12月收治的近视手术治疗患者63例,根据患者手术方式分为FSLASIK组(行FSLASIK手术,31例,62只眼)和SMILE组(行SMILE手术,32例,62只眼),记录术前及术后1周、1个月、3个月的裸眼视力、最佳矫正视力(BCVA)、角膜前表面Q值、角膜层间光密度、屈光度(球镜、柱镜、等效球镜),并进行比较。结果两组进行logMAR转换后的术前裸眼视力和术前BCVA、术前屈光度、术前角膜前表面Q值、术前角膜层间光密度比较差异均无统计学意义(均P>0.05);术后1周、1个月,两组进行logMAR转换后的裸眼视力、角膜层间光密度、角膜层间光密度与术前差值比较差异均无统计学意义(均P>0.05),SMILE组角膜前表面Q值和角膜前表面Q值与术前差值低于FSLASIK组,差异均具有统计学意义(均P<0.05);术后3个月,两组进行logMAR转换后的裸眼视力比较差异无统计学意义(P>0.05),SMILE组角膜层间光密度、角膜前表面Q值、角膜层间光密度和角膜前表面Q值与术前差值低于FSLASIK组,差异均具有统计学意义(均P<0.05);术后3个月两组球镜屈光度、柱镜屈光度、等效球镜比较差异均无统计学意义(均P>0.05)。术后1周、1个月、3个月,两组角膜前表面Q值的术前术后差值和角膜层间光密度的术前术后差值均无相关性(均P>0.05)。结论SMILE与FSLASIK治疗近视均有较好效果,但SMILE手术对患者角膜层间光密度、角膜前表面Q值的影响更小。
Objective To analyse the effects of small incision lenticule extraction(SMILE)and femtosecond laser-assisted laser in situ keratomileusis(FS-LASIK)on corneal interlayer optical density and Q-value of corneal anterior surface in myopic patients.Methods A retrospective case study.A total of 63 patients with myopia who underwent surgery from July 2023 to December 2023 were selected and divided into the FS-LASIK group(31 cases,62 eyes)and SMILE group(32 cases,62 eyes)according to the surgical method.The uncorrected visual acuity,best corrected visual acuity,Q-value of corneal anterior surface,corneal interlayer optical density,and dioptre(spherical,cylindrical,and spherical equivalent)before and at 1 week,1 month,and 3 months after the surgery were compared between the two groups.Results There was no significant difference in preoperative uncorrected visual acuity and preoperative best corrected visual acuity after logMAR conversion,preoperative dioptre,preoperative Q-value of the corneal anterior surface,and preoperative corneal interlayer optical density between the two groups(P>0.05).There was no significant difference in uncorrected visual acuity after logMAR conversion,corneal interlayer optical density,and the difference between the postoperative and preoperative corneal interlayer optical density between the two groups at 1 week and 1 month after the surgery(P>0.05).The Q-value of the corneal anterior surface and the difference between postoperative and preoperative Q-value of corneal anterior surface in the SMILE group were lower than that in the FS-LASIK group,and the difference was statistically statistically significant(P<0.05).At 3 months after the surgery,there was no significant difference in uncorrected visual acuity after logMAR conversion between the two groups(P>0.05).The corneal interlayer optical density,the Qvalue of the corneal anterior surface,and the difference between postoperative and preoperative corneal interlayer optical density and Q-value of the corneal anterior surface in the SMILE group were lower than those in the FS-LASIK group,and the difference was statistically significant(P<0.05).There was no significant difference in spherical dioptre,cylindrical dioptre,and spherical equivalent between the two groups at 3 months after the surgery(P>0.05).There is no correlation between the difference between postoperative and preoperative corneal interlayer optical density and Q-value of corneal anterior surface at 1 week,1 month and 3 months after the surgery(P>0.05).Conclusions Both SMILE and FS-LASIK are effective in the treatment of myopia,but SMILE has a smaller impact on the corneal interlayer optical density and the Q-value of corneal anterior surface.
