摘要
目的 探讨超声引导下膝下动脉逆行腔内介入治疗下肢动脉硬化闭塞症的效果。方法 60例下肢动脉硬化闭塞症患者随机分为两组。对照组采用超声引导下支架植入术,研究组采用超声引导下膝下动脉逆行腔内介入术。比较两组的围术期指标、术后疼痛程度、血管通畅率、 ABI、下肢神经传导速度及足背动脉血流动力学指标、并发症。结果 研究组的手术用时、术后恢复行走及住院时间较对照组短,且术中出血量、术后1 d、 3 d的VAS评分低于对照组(P<0.05)。研究组术后3个月、 6个月的血管通畅率高于对照组;术后1个月、 3个月、 6个月研究组的ABI高于对照组(P<0.05)。术后3个月研究组的下肢MNCV、SNCV、 D、 BF、 PV高于对照组(P<0.05)。研究组的并发症发生率低于对照组(P<0.05)。结论 超声引导下膝下动脉逆行腔内介入治疗下肢动脉硬化闭塞症的手术用时短,术后疼痛刺激轻,可改善足背动脉血流动力学,减轻周围神经损伤,近期效果理想且安全性高。
Objective To explore the effect of ultrasound-guided retrograde endovascular intervention of popliteal artery for treating lower extremity arteriosclerotic occlusive disease.Methods Sixty patients with lower extremity arteriosclerotic occlusive disease were randomly divided into two groups.The control group underwent ultrasound-guided stent implantation,while the study group underwent ultrasound-guided retrograde endovascular intervention of popliteal artery.The perioperative indicators,postoperative pain degree,vascular patency rate,ABI,lower extremity nerve conduction velocity and blood flow dynamics of the dorsal foot artery,as well as complications were compared between the two groups.Results The operation time,postoperative walking recovery time,and hospital stay of the study group were shorter than those of the control group;the intraoperative blood loss and VAS scores at 1 day and 3 days after surgery were lower than those in the control group(P<0.05).The vascular patency rates of the study group were higher than thoset of the control group at 3 months and 6 months after the operation;the ABI of the study group were higher than those of the control group at 1 month,3 months and 6 months after the operation(P<0.05).At 3 months after the operation,the lower limb MNCV,SNCV,D,BF and PV of the study group were higher than those of the control group(P<0.05).The incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusions Ultrasound-guided retrograde endovascular intervention of popliteal artery for treating lower extremity arteriosclerotic occlusive disease is associated with shorter surgical duration,less postoperative pain,which can improve dorsalis pedis artery hemodynamics,alleviate peripheral nerve injury,with favorable short-term outcomes and high safety.
